摘要
目的:探讨正中神经吻合术联合吻合口周围注射自体富血小板血浆(PRP)治疗正中神经损伤的临床效果。方法:选取2019年1月-2020年1月沈阳医学院附属中心医院收治的腕部周围正中神经损伤患者82例为研究对象。按照治疗方式的不同进行分组,对照组41例采用常规正中神经吻合术治疗,观察组41例在常规正中神经吻合术治疗后配合进行吻合口周围注射自体PRP治疗,比较两组治疗效果。结果:治疗后1、3、6、12个月,观察组疼痛(VAS)评分均低于对照组,胰岛素样生长因子1(IGF-1)、血管内皮生长因子(VEGF)均高于对照组,感觉神经传导速度(SNCV)、运动神经传导速度(MNCV)均优于对照组,差异均有统计学意义(P<0.05);治疗后12个月,观察组肌电图总有效率高于对照组(P<0.05)。结论:在手术治疗的基础上予以正中神经损伤患者吻合口周围注射自体PRP治疗可进一步促进患者病情恢复,具有临床推广价值。
Objective:To investigate the clinical effect of median nerve anastomosis combined with injection of autologous platelet-rich plasma (PRP) around anastomosis in the treatment of median nerve injury.Method:A total of 82 patients with median nerve injury around the wrist who admitted to Central Hospital Affiliated to Shenyang Medical College from January 2019 to January 2020 were selected as the research objects.They were grouped according to different treatment methods,41 cases in the control group were treated with conventional median nerve anastomosis,41 cases in the observation group were treated with conventional median nerve anastomosis combined with injection autologous PRP around anastomosis,and the treatment effects of two groups were compared.Result:At 1,3,6 and 12 months after operation,the pain (VAS) scores of the observation group were lower than those of the control group,the levels of insulin-like growth factor 1 (IGF-1) and vascular endothelial growth factor (VEGF) were higher than those of the control group,the sensory nerve conduction velocity (SNCV) and motor nerve conduction velocity (MNCV) were better than those of the control group,the differences were statistically significant (P<0.05).At 12 months after operation,the total effective rate of electromyography in the observation group was higher than that in the control group (P<0.05).Conclusion:On the basis of surgical treatment,injection autologous PRP around anastomosis in patients with median nerve injury can further promote the recovery of the patient’s condition and has clinical promotion value.
