摘要
目的评价多项举措减少静脉用药集中调配差错率,提高调配的质量和速度的实施成效。方法我院PIVAS通过加强人员培训、升级成品输液标签、制定需先用灭菌注射用水溶解的品种参照表、及时登记调剂品种并设立质量监督小组等举措,保障静脉用药调配质量。通过探索增加难溶性药物溶解度的方法和制定部分化疗药物剂量-体积换算表等方式提升静脉用药调配效率。分别比较2018年1~6月与2019年1~6月调配差错率、因差错造成的药品浪费情况,2019年1~3月与2019年4~6月部分化疗药的调剂速率,评价举措实施成效。结果多措并举后,混合调配差错率由0.17‰下降到0.02‰。注射用曲妥珠单抗的调剂速率由5.26 min/组提升至2.64 min/组;紫杉醇注射液的调配速率由3.44 min/组提升至2.04 min/组,差异均具统计学意义。结论我院PIVAS针对混合调配实施的多项举措,对降低调配差错,提高调配质量和速度具有积极促进作用。
OBJECTIVE Evaluate the effect of multiple measures to reduce the error rate of the centralized deployment of intravenous drugs and improve the quality and speed of the deployment.METHODS The quality of drug dispensing was ensured by strengthening personnel training,upgrading the label of finished product infusion,formulating the reference list of varieties to be dissolved with sterilized injection water first,registering the varieties of dispensing timely and setting up a quality supervision group.The ways to increase the solubility of insoluble drugs and develop dose-volume conversion tables for some chemotherapy drugs,were explored to improve the efficiency of drug dispensing.The error rate and waste of drug dispensing from January to June in 2018 and 2019,and the dispensing rate of some chemotherapy drugs from January to March and from April to June in 2019,were compared to evaluate the implementation effect of the measures.RESULTS The error rate was declined from 0.17 to 0.02,the speed rate of herceptin dispensing increased from 5.26 min/group to 2.64 min/group,while the speed rate of paclitaxel injection dispensing increased from 3.44 min/group to 2.04 min/group,after the implementation of the measures.And the differences were statistically significant.CONCLUSION The measures for drug dispensing in PIVAS of our hospital,have a positive effect on reducing errors and improving the quality and speed of drug dispensing.
