摘要
目的:比较腮腺区域切除术与腮腺浅叶切除术治疗腮腺良性肿瘤患者的效果。方法:回顾性分析2019年3月至2021年3月该院收治的62例腮腺良性肿瘤患者的临床资料,根据手术方式不同分为对照组和研究组各31例。对照组采取传统腮腺浅叶切除术治疗,研究组采取腮腺区域切除术治疗。比较两组患者手术相关指标水平、术后面神经损伤程度、术后并发症发生情况及健康状况调查简表(SF-36)评分。结果:研究组患者手术时间、切口长度短于对照组,术中出血量少于对照组,术后面神经损伤程度轻于对照组,差异均有统计学意义(P<0.05);研究组术后面神经损伤总发生率(45.16%)低于对照组(83.87%),术后并发症发生率(12.90%)也低于对照组(41.94%),差异均有统计学意义(P<0.05);术后3个月,研究组患者SF-36各维度评分均显著高于对照组,差异有统计学意义(P<0.05)。结论:与腮腺浅叶切除术相比,腮腺区域切除术治疗腮腺良性肿瘤能够有效减小创伤,缩短手术时间,降低患者术后面神经损伤程度和并发症发生率,并提高患者术后生命质量。
Objective:To compare effects of regional parotidectomy and superficial parotidectomy in treatment of benign parotid tumors.Methods:The clinical data of 62 patients with benign parotid tumors admitted to our hospital from March 2019 to March 2021 were retrospectively analyzed,and they were divided into control group and study group according to different surgical methods,31 cases in each group.The control group was treated with traditional superficial parotidectomy,while the study group was treated with regional parotidectomy.The operation-related indicator levels,the postoperative facial nerve injury degree,the postoperative complications,and the MOS short-form health status survey(SF-36)score were compared between the two groups.Results:The operation time and the incision length of the study group were shorter than those of the control group;the intraoperative blood loss was less than that of the control group;the postoperative facial nerve injury degree was less than that of the control group;and the differences were statistically significant(P<0.05).The total incidence of postoperative facial nerve injury in the study group(45.16%)was lower than that in the control group(83.87%);the incidence of postoperative complications(12.90%)was also lower than that in the control group(41.94%);and the differences were statistically significant(P<0.05).3 months after the surgery,the score of SF-36 in the study group was significantly higher than that in the control group,and the difference was statistically significant(P<0.05).Conclusions:Compared with superficial parotidectomy,the regional parotidectomy for the benign parotid tumors can effectively reduce the trauma,shorten the operation time,reduce the postoperative facial nerve injury degree and the risk of complications,and improve the postoperative quality of life.
