摘要
目的分析干化学与湿化学分析系统常规生化检验项目结果的可比性。方法参照CLSI(美国临床实验室标准化协会)提出的EP15-A2要求,选取2个血清样本,并利用斯马特SD1全自动干式生化分析系统、贝克曼库尔特AU680全自动生化分析系统检测精密度。实验系统为斯马特SD1全自动干式生化分析系统,对比系统为AU680全自动生化分析系统。再测试40份新鲜血清样本,回归分析两个系统的常规生化检验项目,获得r值以及回归方程。结果贝克曼库尔特AU680与斯马特SD1全自动干式生化分析系统的中间精密度较实验室要求更低。所有14项生化检验结果均具有良好的相关性。就医学决定值的偏移而言,ALB(白蛋白)、AST(天冬氨酸转氨酶)、UA(尿酸)、GLU(葡萄糖)、tCO_(2)(二氧化碳总量)、Ca(钙)可接受;CREA(肌酐)、LDH(乳酸脱氢酶)不可接受;CK(肌酸激酶)、UREA(尿素氮)部分接受。结论斯马特SD1干化学、湿化学分析系统大多数生化检验项目结果具有良好的相关性,但是依然存在不可接受的偏移项目,需要临床进一步细化参考区间,以满足临床检验需要。
Objective To analyze the comparability of routine biochemical test results between dry chemical analysis system and wet chemical analysis system.Methods According to the requirements of ep15-A2 proposed by CLSI(American Association for clinical laboratory standardization),two serum samples were selected,and the precision was detected by smart SD1 automatic dry biochemical analysis system and Beckman Kurt AU680 automatic biochemical analysis system.The experimental system is smart SD1 automatic dry biochemical analysis system,and the comparison system was AU680 automatic biochemical analysis system.Then 40 fresh serum samples were tested,and the routine biochemical test items of the two systems were analyzed by regression,and the r value and regression equation were obtained.Results The intermediate precision of Beckman Coulter AU680 and Smart SD1 automatic dry biochemical analysis system was lower than that of laboratory.Among the 14 biochemical indexes,ALB、AST(aspartate aminotransferase),UA(uric acid),GLU(glucose),tCO_(2)(total carbon dioxide)and Ca were acceptable in terms of the deviation of medically determined values.CREA(creatinine),LDH(lactate dehydrogenase)unacceptable;CK,UREA nitrogen were partially accepted.Conclusion There is a good correlation between the results of routine biochemical tests of smart SD1 dry chemistry and wet chemistry analysis system,but there are still unacceptable deviation items,which need to further refine the reference interval to meet the needs of clinical testing.
