重度中暑相关血管内皮细胞损伤的机制及研究进展

摘要 重度中暑是指暴露于炎热环境、剧烈运动所致的机体产热与散热失衡,以机体内核心温度>40℃和中枢神经系统异常为特征,如精神状态改变、抽搐或昏迷,易引起全身炎症反应综合征、弥散性血管内凝血和多脏器功能衰竭的临床综合征。研究显示,血管内皮细胞是重度中暑时重要的效应细胞和靶细胞,也是极易发生损伤的细胞,其损伤机制主要包括热应激、炎症反应、氧化应激和肠道功能紊乱。血管内皮细胞功能障碍在重度中暑的发生和发展中起着至关重要的作用,然而,重度中暑造成的血管内皮细胞损伤的确切机制目前尚不完全清楚。本篇综述就重度中暑相关血管内皮细胞损伤的机制及最新的研究进展作简单的介绍,让人们对重度中暑有更深入的理解。 Severe heatstroke refers to the imbalance of heat production and heat dissipation caused by exposure to hot environment and vigorous exercise,which is characterized by the increase of core temperature in the machine over 40℃ and the abnormality of central nervous system,such as mental state change,convulsion or coma,it is easy to cause systemic inflammatory response syndrome(SIRS),disseminated intravascular coagulation(DIC) and clinical syndrome of multiple organ dysfunction syndrome(MODS).Studies have shown that vascular endothelial cells are important effector cells and target cells during severe heat stroke,and they are also cells that are vulnerable to damage.The damage mechanisms mainly include heat stress,inflammatory response,oxidative stress and intestinal dysfunction.Vascular endothelial cell dysfunction plays a crucial role in the occurrence and development of severe heatstroke,however,the exact mechanism of vascular endothelial cell damage caused by severe heatstroke is not fully understood.This review briefly introduces the mechanism and latest research progress of vascular endothelial cell injury associated with severe heatstroke,and provides a deeper understanding of severe heatstroke.
