摘要
目的比较一次性根管治疗与多次法根管治疗牙体牙髓病的效果。方法选取本院2017年9月至2020年11月收治的88例牙体牙髓病患者,采用随机数字表法将其分为A组和B组,各44例。A组接受多次法根管治疗,B组接受一次性根管治疗。比较两组的应激指标、炎性因子水平、咬合性疼痛及自发性疼痛评分、临床疗效及并发症发生情况。结果治疗后3 d,两组的血清NE、E、Cor水平均较治疗前明显升高,但B组低于A组(P<0.05)。治疗后3 d,两组的血清IL-2、IL-6、TNF-α水平均较治疗前显著降低,且B组低于A组(P<0.05)。治疗后7 d,两组的咬合性疼痛及自发性疼痛评分均较治疗前升高,但B组低于A组(P<0.05)。B组的治疗总有效率高于A组,并发症总发生率显著低于A组(P<0.05)。结论与多次法根管治疗相比,给予牙体牙髓病患者一次性根管治疗可显著减轻应激反应、炎症反应及术后疼痛,整体疗效更佳,安全性更高。
Objective To compare the effect of one-time root canal therapy and multiple root canal therapy on dental pulp disease.Methods Eighty-eight patients with dental pulp disease treated in our hospital from September 2017 to November 2020 were selected and divided into group A and group B by random number table method,with 44 cases in each group.The group A received multiple root canal therapy,and the group B received one-time root canal therapy.The stress indexes,inflammatory factors level,occlusal pain and spontaneous pain scores,clinical efficacy and complications were compared between the two groups.Results At 3 d after treatment,the levels of serum NE,E and Cor in the two groups were significantly higher than those before treatment,but those of the group B were lower than the group A(P<0.05).At 3 d after treatment,the levels of serum IL-2,IL-6 and TNF-α in the two groups were significantly lower than those before treatment,and those of the group B were lower than the group A(P<0.05).At 7 d after treatment,the occlusal pain and spontaneous pain scores of the two groups were higher than those before treatment,but those of the group B were lower than the group A(P<0.05).The total effective rate of treatment in the group B was higher than that in the group A,and the total incidence of complications was significantly lower than that in the group A(P<0.05).Conclusion Compared with multiple root canal therapy,one-time root canal therapy for patients with dental pulp disease can significantly reduce stress response,inflammatory response and postoperative pain,with better overall curative effect and higher safety.
