摘要
目的观察高分辨率电子计算机断层扫描(HRCT)定量参数磨玻璃成分CT值(CTGGO)、标准化摄取值(SUV)水平与早期肺腺癌浸润程度的相关性。方法回顾性分析75例早期肺腺癌患者资料,所有患者均在术前接受HRCT检查,查阅患者资料统计手术切除肺磨玻璃结节(GGO)个数,统计所有GGO浸润程度,根据结果将不典型腺瘤样增生(AAH)、原位腺癌(AIS)纳入浸润前组,将微浸润腺癌(MIA)纳入MIA组,将浸润性腺癌(IAC)纳入IAC组。设计基线资料填写表,详细填写3组GGO的HRCT影像学资料,分析HRCT定量参数CTGGO、SUV水平与早期肺腺癌浸润程度的关系。结果75例早期肺腺癌患者手术共切除86个GGO,其中GGO为AAH 6个、AIS 12个,纳入浸润前组;GGO为MIA 44个,纳入MIA组;GGO为IAC 24个,纳入IAC组;IAC组GGO的HRCT定量参数CTGGO、SUV水平均高于浸润前组与MIA组,差异有统计学意义(P<0.05);浸润前组与MIA组的GGO的HRCT定量参数CTGGO、SUV水平比较,差异无统计学意义(P>0.05);3组间其他HRCT影像学资料比较,差异无统计学意义(P>0.05);经Kendall's tau-b相关性分析,GGO的HRCT定量参数CTGGO、SUV水平与早期肺腺癌浸润程度呈正相关(γ>0,P<0.05);将IAC浸润定义为状态变量,绘制ROC曲线结果显示,GGO的HRCT定量参数CTGGO、SUV水平单独及联合预测IAC浸润的AUC均>0.80,预测价值均较理想,且以联合预测价值最佳。结论早期肺腺癌患者GGO HRCT定量参数CTGGO、SUV值与浸润程度有关,随着两者检测值升高,浸润程度增加,且将两者用于预测IAC有一定应用价值。
Objective To observe the correlation between ground glass component CT value(CTGGO)and standardized uptake value(SUV)of high resolution computed tomography(HRCT)and the invasive degree of early lung adenocarcinoma.Methods The data of 75 patients with early lung adenocarcinoma were retrospectively analyzed,all patients underwent HRCT examination before operation,the data of patients were consulted,the number of surgical removal of pulmonary ground glass nodules(GGO)was counted,the invasive degree of all GGO was counted,according to the results,atypical adenomatous hyperplasia(AAH)and adenocarcinoma in situ(AIS)were included in the preinvasive group,microinvasive adenocarcinoma(MIA)in the MIA group,and invasive adenocarcinoma(IAC)in the IAC group.Designed the baseline data filling form,HRCT imaging data of three groups of GGO were filled in detail,the relationship between HRCT quantitative parameters CTGGO,SUV levels and the invasive degree of early lung adenocarcinoma was analyzed.Results In 75 patients with early lung adenocarcinoma,a total of 86 GGOS were removed in surgery,of which 6 GGO were AAH and 12 were AIS,which were included in the pre invasion group;44 GGO were MIA,which were included in the MIA group;24 GGO were IAC,which were included in the IAC group;the HRCT quantitative parameters CTGGO and SUV of GGO in IAC group were significantly higher than those in pre invasion group and MIA group,there was statistical significant difference(P<0.05);the HRCT quantitative parameters CTGGO and SUV of GGO were compared between pre invasion group and MIA group,there was no statistical significant difference(P>0.05);compared other HRCT imaging data among the 3 groups,there was no statistical significant difference(P>0.05);Kendall's tau-b correlation analysis showed that the levels of CTGGO and SUV were positively correlated with the invasive degree of early lung adenocarcinoma(γ>0,P<0.05);IAC infiltration was defined as a state variable,ROC curve was drawn,the results showed that the AUC of HRCT quantitative parameters CTGGO and SUV levels of GGO alone or combined in predicting IAC invasion were>0.80,the predictive value was ideal,and the combined prediction value was the best.Conclusion The quantitative parameters CTGGO and SUV of GGO HRCT in patients with early lung adenocarcinoma are related to the degree of invasion,with the increase of the detection value of both,the degree of infiltration is increased,and it has certain value to use them in predicting IAC.
