摘要
目的了解核电生产企业员工甲状腺患病率情况,为核电生产企业员工职业健康防护提供科学依据。方法选取2020年5—11月广西某核电生产企业909名员工职业健康检查甲状腺超声及甲状腺功能血清学指标检测结果,根据甲状腺超声诊断结果分为甲状腺正常组、甲状腺结节组和甲状腺弥漫性病变组进行分析。结果该核电企业909名员工检出甲状腺疾病250人,总检出率为27.50%,其中男性236人,检出率27.03%,女性14人,检出率38.89%,男女检出率差异无统计学意义(P>0.05)。超声检查甲状腺异常209人,异常检出率为22.99%,其中甲状腺结节171人,检出率18.81%,甲状腺弥漫性病变38人,检出率4.18%,甲状腺结节检出率明显高于甲状腺弥漫性病变,差异有统计学意义(P<0.01)。甲状腺功能血清学指标异常66人,异常检出率为7.26%。甲状腺弥漫性病变组甲状腺功能血清学指标促甲状腺素水平及甲状腺功能指标异常率均明显高于甲状腺正常组和甲状腺结节组,差异均有统计学意义(均P<0.05)。结论该核能发电企业生产环境辐射本底剂量在安全范围以内,对作业人员甲状腺患病率影响较小。应重点关注超声甲状腺异常及甲状腺功能异常者,尤其是甲状腺弥漫性病变合并促甲状腺素水平升高者。
Objective To understand the prevalence of thyroid in employees of nuclear power production enterprises,and to provide scientific basis for occupational health protection of employees of nuclear power production enterprises.Methods The results of thyroid ultrasound and thyroid function serological index in occupational health examination of 909 employees in a nuclear power production enterprise in Guangxi Province were selected from May to November 2020.According to the results of thyroid ultrasound diagnosis,they were divided into normal thyroid group,thyroid nodule group and thyroid diffuse lesion group for analysis.Results Among the 909 employees,250 were diagnosed with thyroid disease,and the total detection rate of 27.50%,including 236 males with a detection rate of 27.03%and 14 females with the rate of 38.89%.There was no significant difference in the detection rate between males and females(P>0.05).There were 209 cases of thyroid abnormality examined by ultrasound,and the detection rate was 22.99%,including 171 cases of thyroid nodules(18.81%)and 38 cases of diffuse thyroid lesions(4.18%).The detection rate of thyroid nodules was significantly higher than that of diffuse thyroid lesions,and the difference was statistically significant(P<0.01).There were 66 patients with abnormal thyroid function,and the detection rate was 7.26%.The level of thyroid stimulating harmony(TSH)and the abnormal rate of thyroid function in the diffuse thyroid disease group were significantly higher than those in the normal thyroid group and the thyroid nodule group,with statistical significance(all P<0.05).Conclusion The radiation background dose in the production environment of the nuclear power plant is within the safe range,which has little influence on the thyroid prevalence rate of the workers.However,attention should be paid to patients with thyroid abnormalities and thyroid function abnormality,especially those with diffuse thyroid lesions combined with elevated TSH level.
