摘要
目的 探究影响胎儿生长受限(FGR)的危险因素及分娩孕周对围产儿结局的影响。方法 回顾性分析588例FGR孕妇的临床资料,分析影响FGR的危险因素。产后追踪610例FGR围产儿相关情况,分析围产儿的结局。结果 孕期增重不足、妊娠期高血压、妊娠期糖尿病、脐血流异常、羊水过少和双胎妊娠与FGR相关(P<0.05)。多因素logistic回归分析结果显示,孕期增重不足、妊娠期高血压、脐血流异常、羊水过少和双胎妊娠是影响FGR的独立危险因素(P<0.05)。不同分娩孕周FGR围产儿的Apgar评分、体重和不良结局总发生率均有统计学差异(P<0.05);随着分娩孕周增大,Apgar评分逐渐增高,不良结局总发生率逐渐下降(P<0.05)。结论 孕期增重不足、妊娠期高血压、脐血流异常、羊水过少和双胎妊娠是影响FGR的独立危险因素。分娩孕周是影响FGR围产儿预后的关键因素。
Objective To explore the risk factors of fetal growth restriction(FGR) and the effect of gestational age at delivery on the perinatal outcomes.Methods The clinical data of 588 pregnant women with FGR were retrospectively analyzed, and the risk factors for FGR were analyzed.A total of 610 FGR perinatal infants were followed up after delivery.The perinatal outcomes were analyzed.Results Insufficient weight gain during pregnancy, gestational hypertension, gestational diabetes mellitus, abnormal umbilical cord blood flow, oligohydramnios and twin pregnancy are related to FGR(P<0.05).Multivariate logistic regression analysis showed that insufficient weight gain during pregnancy, pregnancy hypertension, abnormal umbilical cord blood flow, oligohydramnios and twin pregnancy were the independent risk factors for FGR(P<0.05).There were significant differences in Apgar score, weight and the total incidence of adverse outcomes among FGR perinatal infants at different gestational ages at delivery(P<0.05).With the increase of gestational age at delivery, the Apgar score gradually increased and the total incidence of adverse outcomes gradually decreased(P<0.05).Conclusion Insufficient weight gain during pregnancy, pregnancy hypertension, abnormal umbilical cord blood flow, oligohydramnios and twin pregnancy are the independent risk factors for FGR.Gestational age at delivery is a key factor affecting the prognosis of the perinatal infants with FGR.
