摘要
目的:探讨用抗早颗粒加减治疗女童性早熟伴乳房发育的效果。方法:回顾性选取江苏省中医院东华病历系统收录的2017年8月至2020年9月期间就诊于该医院生长发育专科叶进教授门诊的100例性早熟伴乳房发育女童的电子病历进行研究。用抗早颗粒加减对这些患儿进行治疗(治疗时间均超过3个月),比较治疗前后其骨龄与实际年龄差值(BA-CA)、子宫和卵巢的体积、卵泡数目、体质指数(BMI)及临床症状积分,同时观察乳房发育类型不同组﹝特发性中枢性性早熟(ICPP)组和单纯乳房早发育(IPT)组﹞患儿及治疗时间不同患儿的上述各项指标。结果:治疗后,100例患儿的子宫体积大于治疗前,临床症状积分低于治疗前,P<0.05。治疗后,100例患儿的乳房明显缩小,乳房的Tanner分期明显低于治疗前,其中有32例患儿的乳房缩小至幼儿水平。治疗后,IPT组患儿的子宫体积大于治疗前,临床症状积分低于治疗前,P<0.05。治疗后,ICPP组患儿的子宫体积大于治疗前,P<0.05。治疗时间为3~6个月的患儿,治疗后其子宫体积大于治疗前,临床症状积分低于治疗前,P<0.05。治疗时间为7~12个月的患儿,治疗后其BA-CA和子宫体积均大于治疗前,P<0.05。结论:用抗早颗粒加减治疗女童性早熟伴乳房发育的效果较好,能显著减轻其临床症状。
Objective:To investigate the effect of kangzao granules on the treatment of girls with precocious puberty and breast development.Methods:The electronic medical records of 100 cases of precocious girls with breast development who visited Professor Ye Jin’s outpatient department of growth and development department of Jiangsu Hospital of Traditional Chinese Medicine from August 2017 to September 2020 were retrospectively selected and studied.The difference between bone age and actual age(BA-CA),uterine and ovarian volume,follicular number,body mass index(BMI)and clinical symptom score were compared before and after treatment with kangzao granules.At the same time,the above indexes were observed in children with different types of breast development(ICPP group and IPT group)and different treatment duration.Results:After treatment,the uterine volume of 100 children was larger than before treatment,and the clinical symptom score was lower than before treatment,P<0.05.After treatment,100 cases of breast were significantly reduced,and the Tanner stage of breast was significantly lower than before treatment,and 32 cases of breast were reduced to infant level.After treatment,the uterine volume of children in IPT group was larger than before treatment,and the clinical symptom score was lower than before treatment,P<0.05.After treatment,the uterine volume of ICPP group was larger than that before treatment,P<0.05.For children with 3-6 months of treatment,the uterine volume after treatment was larger than before treatment,and the clinical symptom score was lower than before treatment,P<0.05.The ba-CA and uterine volume of 7 to 12 months after treatment were higher than before,P<0.05.Conclusion:Kangzao granule has a better effect on the treatment of girls with precocious puberty and breast development,and can significantly reduce its clinical symptoms.
