摘要
目的:比较分析盆腔脓肿腹腔镜手术前不同抗感染治疗时机临床疗效、术后并发症及复发率,为治疗方案选择提供参考。方法:病例资料来源于2018年6月-2021年2月本院妇科收治的盆腔脓肿患者共89例,根据腹腔镜手术时机分为早期组(47例,抗感染治疗3 d后实施手术)和延迟组(42例,抗感染治疗7 d后实施手术),两组麻醉、手术方法一致,术后均继续给予抗生素治疗。记录手术完成情况及住院费用,统计术后并发症发生率及盆腔脓肿复发率。结果:早期组手术持续时间、术后体温恢复正常时间、白细胞恢复正常时间、排气时间、总住院时间均短于延迟组,术中出血量、总住院费用低于延迟组(均P<0.05);两组术后周围脏器(输尿管、膀胱、肠道)损伤,盆腔疼痛,切口感染等并发症发生率无差异(P>0.05)。门诊随访6~12个月,早期组盆腔积脓复发率(4.3%)、输卵管积脓复发率(2.1%)均低于延迟组(19.1%、14.3%)(均P<0.05)。结论:抗感染治疗3 d后实施腹腔镜手术治疗盆腔脓肿疗效确切,且术后恢复快,不增加并发症风险,复发率低,并减少了住院时间和费用。
Objective:To compare and analyze the curative effects and the rates of postoperative complications and recurrence of different timing of laparoscopic surgery for treating pelvic abscess after anti-infective therapy,and to provide reference for the optimal treatment options of pelvic abscess.Methods:The data of 89 patients with pelvic abscess from June 2018 to February 2021 were collected.These patients were divided into group A and group B.47 patients in group A were given laparoscopic surgery of pelvic abscess after 3 days of anti-infective therapy,and 42 patients in group B were given laparoscopic surgery of pelvic abscess after 7 days of anti-infective therapy treatment.The anesthesia and surgical methods of the patients in the two groups were consistent,and the patients in the two groups were continuously given anti-infective therapy after operation.The surgery situation and hospitalization expenses of the patients in the two groups were recorded.The incidences of postoperative complications and recurrence of pelvic abscess of the patients in the two groups were counted.Results:The duration of operation,the time of temperature returned to normal,the time of temperature WBC value returned to normal,the exhaust time,and the total hospital stay time of the patients in group A were significantly shorter than those of the patients in group B,and the amount of intraoperative blood loss and the total hospitalization expenses of the patients in group A were significantly lower than those of the patients in group B(P<0.05).There were no significant differences in the incidences of postoperative complications,such as injury of peripheral organs(ureter,bladder,and intestine),pelvic pain,and incision infection,of the patients between the two groups(P>0.05).The recurrence rates of pelvic abscess(4.3%)and fallopian tube abscess(2.1%)of the patients in group A were significantly lower than those(19.1%and 14.3%)of the patients in group B during 6-12 months of outpatient followed up(P<0.05).Conclusion:laparoscopic surgery for treating pelvic abscess after 3 days of anti-infective treatment has exact curative effect and rapid recovery after operation,which does not increase the risk of complications and recurrence of pelvic abscess,and can control hospitalization cost effectively.
