摘要
目的探讨羟苯磺酸钙联合替米沙坦治疗早期糖尿病肾病(DN)的临床疗效,以及对患者血管内皮功能、肾功能和炎性因子水平的影响。方法选取湖北省应城市人民医院2018年11月至2020年11月收治的早期DN患者100例,按随机数字表法分为对照组和观察组,各50例。两组患者均予对症治疗+替米沙坦,观察组患者加用羟苯磺酸钙,均连续治疗12周。结果观察组总有效率为94.00%,显著高于对照组的80.00%(P<0.05)。与对照组比较,观察组患者治疗后的内皮素-1、尿素氮、24 h尿白蛋白排泄率、血肌酐、β2-微球蛋白、白细胞介素6及肿瘤坏死因子-α水平均显著降低(P<0.05),一氧化氮水平显著升高(P<0.05);观察组和对照组治疗期间的不良反应发生率相当(12.00%比10.00%,P>0.05)。结论羟苯磺酸钙联合替米沙坦治疗早期DN的临床疗效较好,可改善患者的血管内皮功能和肾功能,减轻炎性反应,且治疗安全性良好。
Objective To investigate the clinical efficacy of calcium dobesilate combined with telmisartan in the treatment of early diabetic nephropathy(DN),and the effects on patients'vascular endothelial function,renal function and inflammatory factors.Methods A total of 100 patients with early DN admitted to People's Hospital of Yingcheng from November 2018 to November 2020 were selected and divided into the control group and the observation group according to the random number table method,with 50 cases in each group.The patients in the two groups were treated with symptomatic treatment and telmisartan,on this basis,the patients in the observation group were treated with calcium dobesilate.Both groups were treated for 12 consecutive weeks.Results The total effective rate of the observation group was 94.00%,which was significantly higher than 80.00%of the control group(P<0.05).Compared with those in the control group,the levels of endothelin-1(ET-1),blood urea nitrogen(BUN),24 h urinary albumin excretion rate(UAER),serum creatinine(SCr),β2-microglobulin(β2-MG),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)were decreased significantly in the observation group(P<0.05),and the level of nitric oxide(NO)was increased significantly in the observation group(P<0.05).During the treatment,the incidence of adverse reactions in the observation group was similar to that in the control group(12.00%vs.10.00%,P>0.05).Conclusion Calcium dobesilate combined with telmisartan is effective and safe in the treatment of early DN,which can improve vascular endothelial function and renal function,and reduce inflammatory reaction.
