摘要
目的调查天津市滨海新区某饭店感染性腹泻暴发疫情的感染来源、传播途径和危险因素.方法按照病例定义开展病例搜索,采用描述性流行病学方法进行分析,样本采用RT-PCR方法检测诺如病毒核酸.结果2019年5月11日起共发生感染性腹泻病例172例,其中就餐人员164例,饭店员工8例;临床症状主要为腹泻(97.79%)、呕吐(69.77%)和发热(55.81%);年龄中位数为42岁,年龄范围为7-78岁.首例员工发病于5月11日4:00时,首例顾客发病于5月13日12:00时,5月14日12:00-5月15日00:00时前为发病高峰,末例发病于5月16日20:00时.共采集标本53份,包括43份便标本(40份患者、3份酒店员工)、9份环境标本、1份呕吐物标本,其中39份粪便标本诺如病毒核酸阳性,其余常规肠道致病菌检测阴性;其中可疑餐次发生在5月13日,此日罹患率明显高于其他时间(P<0.05),且病例老醋蜇头暴露率高于5月14日(χ^(2)=2.32,P<0.05).结论这是1起由诺如病毒引起的感染性腹泻暴发,暴发原因可能是携带病毒的厨工污染部分食物所致,主要可疑传播食物为凉菜老醋蛰头.
Objective To investigate the source of infection,transmittable path and risk factors of the outbreak of infectious diarrhea in Tianjin Binhai New Area's restaurant.Methods To search cases according to case definition,analyze data using descriptive epidemiology,and examine norovirus using RT-PCR.Resu Its There were 172 infectious diarrhea cases from May 11th 2019 including 164 cases eating at that restaurant and 8 cases working in that restaurant;clinical symptoms mainly included diarrhea(97.79%),vomiting(69.77%),fever(55.81%).The age median was 42 years,and the range between the minimum and maximum was 7-78 years old.The first worker case occurred at 4:00,May ll^(th),and the first customer case occurred atl2:00,May 12^(th).The incidence peak was between 12:00 May 14th and 0:00 May 15th,and the suspicious food was Old Vinegar head.The last case occurred at 20:00.53 specimens were collected,including 43 stool specimens(40 sick men's and 3 workers'),9 environment specimens,1 vomiting specimen.Above the 53 specimens,39 stool specimens were examined nucleic acid of norovirus positive,and the rest were examined negative.The incidence rate of the suspicious meal in May 13lh was dramatically higher than the others(P<0.05),and the exposure rate of cases eating Old Vinegar head was higher than in May 14,h(χ^(2)=2.32,P<0.05).Conelusion This was a outbreak of infectious diarrhea caused by norovirus,the probable reason of this outbreak was that the cook carrying norovirus contaminated part food,and the main food transmitting norovirus was Old Vinegar head by means of foodbome transmission.
