摘要
目的探讨基于5E康复模式的延续护理对尿毒症血液透析患者钙磷代谢的影响。方法选取2019年1月—2021年3月在南阳市某医院进行血液透析治疗的尿毒症患者104例,采用随机数字表法分为延续组和对照组,每组52例。对照组患者采用常规护理,延续组患者在对照组基础上采用基于5E康复模式的延续护理。比较2组患者干预前后肾功能、钙磷代谢水平、自我管理能力及并发症发生情况。结果2组患者干预后血肌酐(Scr)及尿素氮(BUN)水平低于干预前,且延续组低于对照组,差异均有统计学意义(P<0.05)。2组患者干预后血钙水平高于干预前且延续组高于对照组,血磷水平低于干预前且延续组低于对照组,差异均有统计学意义(P<0.05)。2组患者干预后自我管理评分高于干预前,且延续组高于对照组,差异均有统计学意义(P<0.05)。延续组患者并发症发生率为3.85%(2/52),低于对照组的11.54%(6/52),但差异无统计学意义(χ^(2)=1.219,P=0.270)。结论基于5E康复模式的延续护理用于尿毒症血液透析患者中,有助于提高患者自我管理能力,改善患者肾功能与钙磷代谢,预防并发症。
Objective To explore the effect of continuing care based on the 5E rehabilitation model on calcium and phosphorus metabolism in uremic hemodialysis patients.Methods One hundred and four patients with uremia treated with hemodialysis in a hospital in Nanyang from January 2019 to March 2021 were selected and divided into continuing group and control group using the random number table method,with 52 cases in each group.Patients in the control group received conventional care,and patients in the continuing group were given continuing care based on the 5E rehabilitation model on the basis of the control group.The renal function,calcium and phosphorus metabolism level and self-management ability before and after the intervention,as well as the incidence of complications,were compared between the two groups.Results The serum creatinine(Scr)and blood urea nitrogen(BUN)levels in the 2 groups were lower than before the intervention and lower in the continuing group than in the control group,with statistically significant differences(P<0.05).Blood calcium levels in the 2 groups were higher than before the intervention and higher in the continuing group than in the control group,while blood phosphorus levels were lower than before the intervention and lower in the continuing group than in the control group,all with statistically significant differences(P<0.05).The self-management scores of the two groups were higher than before the intervention and higher in the continuing group than in the control group,with statistically significant differences(P<0.05).The complication rate of patients in the continuing group was 3.85%(2/52),which was lower than that of 11.54%(6/52)in the control group,but the difference was not statistically significant(χ^(2)=1.219,P=0.270).Conclusion Continuing care based on the 5E rehabilitation model,when used in uremic hemodialysis patients,can help improve patients′self-management ability,improve their renal function and calcium and phosphorus metabolism,as well as prevent complications.
