摘要
目的分析恶性淋巴瘤患者血流感染情况及病原菌耐药性。方法选取310例恶性淋巴瘤患者,检测血流感染的病原菌分布情况,以纸片扩散法进行药敏反应试验。结果310例恶性淋巴瘤患者中,发生血流感染29例,感染率为9.35%。29例发生血流感染的恶性淋巴瘤患者中共分离出25株病原菌,其中革兰阳性菌15株,占60.00%;革兰阴性菌10株,占40.00%;未检出真菌。表皮葡萄球菌、其他凝固酶阴性葡萄球菌均对氨苄西林/舒巴坦钠、青霉素G耐药,链球菌对氨苄西林/舒巴坦钠耐药;大肠杆菌对庆大霉素耐药,肺炎克雷伯菌对头孢唑林耐药,铜绿假单胞菌对左氧氟沙星耐药。结论恶性淋巴瘤患者存在一定的血流感染风险,应加强病原菌耐药性监测,有助于合理应用抗菌药物。
Objective The bloodstream infection and drug resistance of pathogens in patients with malignant lymphoma were analyzed.Method A total of 310 patients with malignant lymphoma were selected,the distribution of pathogenic bacteria in bloodstream infection was detected,and drug susceptibility test was carried out by disc diffusion method.Result Among 310 patients with malignant lymphoma,29 patients had bloodstream infection,and the infection rate was 9.35%.A total of 25 pathogenic bacteria were isolated from 29 patients with malignant lymphoma with bloodstream infection,including 15 gram-positive bacteria,accounting for 60.00%;10 gram-negative bacteria,accounting for 40.00%;no fungi were detected.Staphylococcus epidermidis and other coagulase-negative staphylococci were resistant to ampicillin/sulbactam sodium and penicillin G,streptococcus was resistant to ampicillin/sulbactam sodium.Escherichia coli was resistant to gentamicin,klebsiella pneumoniae was resistant to cefazolin,and pseudomonas aeruginosa was resistant to levofloxacin.Conclusion There is a certain risk of bloodstream infection in patients with malignant lymphoma,and the monitoring of drug resistance of pathogens should be strengthened to help rational use of antibiotics.
