摘要
目的探讨对比飞秒激光准分子激光原位角膜磨镶术(LASIK)及准分子激光上皮瓣下角膜磨镶术(LASEK)对高度近视患者的临床效果。方法76例(140眼)高度近视患者,随机分为飞秒激光LASIK组和LASEK组,每组38例(70眼)。飞秒激光LASIK组采用美国Intralsae飞秒激光系统和德国蔡司MEL90准分子激光系统实施LASIK,LASEK组采用德国蔡司MEL90准分子激光系统实施LASEK。分析两组患者的术后症状,比较两组患者术后不同时期(术后1 d、1周、1个月、3个月、6个月)视力水平,术后角膜厚度。结果术后第1天,飞秒激光LASIK组仅有轻度症状4眼,发生率为5.7%;LASEK组均存在不同程度的疼痛、异物感、畏光流泪,其中症状突出38眼(54.3%),轻度表现32眼(45.7%)。术后1周,飞秒激光LASIK组未发生角膜刺激症状,LASEK组仅5眼发生角膜刺激症状,发生率为7.1%。术后1周、1个月,飞秒激光LASIK组的视力分别为的(1.06±0.13)、(1.09±0.12),均高于LASEK组的(0.65±0.11)、(0.84±0.14),差异具有统计学意义(P<0.05);术后3、6个月,两组视力水平比较差异无统计学意义(P>0.05)。两组患者术后1、3、6个月的角膜厚度比较差异均无统计学意义(P>0.05)。结论高度近视患者采用飞秒激光LASIK治疗具有安全性高、疗效佳的特征,是较安全稳定的术式。
Objective To discuss the clinical effect of femtosecond-assisted laser in situ keratomileusis(LASIK)and laser-assisted subepithelial keratectomy(LASEK)on patients with high myopia.Methods A total of 76 patients(140 eyes)with high myopia were randomly divided into femtosecond-assisted LASIK group and LASEK group,with 38 cases(70 eyes)in each group.The femtosecond-assisted LASIK group used the American Intralsae femtosecond laser system and the German Zeiss MEL90 excimer laser system to perform LASIK,and the LASEK group used the German Zeiss MEL90 excimer laser system to perform LASEK.The postoperative symptoms of the two groups were analyzed,and the visual acuity level and postoperative corneal thickness were compared between the two groups at different time periods(1 d,1 week,1 month,3 months,and 6 months after surgery).Results At 1 d after surgery,the femtosecond-assisted LASIK group had only 4 eyes with mild symptoms,with an incidence of 5.7%.The LASEK group all had different degrees of pain,foreign body sensation,photophobia and tearing,of which 38 eyes(54.3%)had prominent symptoms and 32 eyes(45.7%)had mild manifestations.At 1 week after surgery,no corneal irritation symptoms occurred in the femtosecond-assisted LASIK group,but only 5 eyes in the LASEK group developed corneal irritation symptoms,with an incidence rate of 7.1%.At 1 week and 1 month after surgery,the visual acuity of the femtosecond-assisted LASIK group were(1.06±0.13)and(1.09±0.12),which were higher than(0.65±0.11)and(0.84±0.14)of the LASEK group,and the differences were statistically significant(P<0.05).At 3 and 6 months after surgery,the differences in visual acuity levels between the two groups were not statistically significant(P>0.05).There was no statistically significant difference in corneal thickness between the two groups at 1,3,and 6 months after surgery(P>0.05).Conclusion The femtosecond-assisted LASIK treatment has the characteristics of high safety and efficacy for patients with high myopia,and it is a relatively safe and stable surgery.
