摘要
目的探究MRI在儿童膝关节损伤诊断中的应用效果。方法郑州大学附属儿童医院、河南省儿童医院、郑州儿童医院医学影像科于2019年6月至2020年12月收治60例膝关节损伤患儿为研究对象,根据诊断方式的不同作为分组标准,以关节镜作为金标准,接受超声检查作为对照组,接受MRI作为观察组,将两组膝关节损伤诊断效果进行比较。结果经过MRI检查,60例患者100个半月板中,其中半月板损伤58例,占比96.67%;MRI检出28例软骨损伤,MRI诊断软骨损伤表现为软骨凹陷和断续,短T1WI信号,长T2WI信号。60例患者共100个半月板,其中关节镜检查出共48个,MRI检查出48个。60例患者共存在100例软骨损伤中,其中关节镜检查共检出19例关节软骨损伤,MRI检出13例例软骨损伤。60例患者的检查结果以关节镜诊疗为金标准,观察组诊断半月板损伤的灵敏度为95.83%(46/48),特异度96.15%(50/52),准确率96.00%(96/100),观察组诊断关节软骨损伤的灵敏度为57.89%(11/19),特异度97.53%(79/81),准确率90.00%(90/100)。结论对于儿童膝关节损伤采用MRI诊断,可获得良好的灵敏度、特异度和准确性,能够为临床上膝关节损伤的诊断治疗提供有价值的参考依据,也是膝关节损伤安全有效的诊断方法。但是需要注意的是,对于损伤程度较轻的膝关节损伤患儿,由于MRI的灵敏度较低,应予以临床重视。
Objective To explore the effect of MRI in the diagnosis of knee joint injury in children.Methods From June 2019 to December 2020,60 cases of children with knee joint injury were treated in children’s Hospital of Zhengzhou University,Children’s Hospital of Henan Province and Zhengzhou Children’s Hospital,according to the different diagnostic methods as the grouping standard,arthroscopy as the gold standard,ultrasound as the control group,MRI as the observation group,the two groups were compared in the diagnosis of knee injuries.Results Among the 100 Menisci of 60 patients,58 cases(96.67%)were menisci injuries,28 cases of cartilage injuries were detected by MRI,which were characterized by cartilage depression and discontinuity,short T1WI signal and long T2WI signal.There were 100 Menisci in 60 patients,of which 48 were detected by arthroscopy and 48 by MRI.There were 100 cartilage injuries in 60 patients,of which 19 were detected by arthroscopy and 13 by MRI.Arthroscopy was the gold standard in 60 patients,in the observation group,the sensitivity,specificity and accuracy of diagnosing meniscus injury were 95.83%(46/48),96.15%(50/52)and 96.00%(96/100),respectively,the sensitivity,specificity and accuracy of diagnosing articular cartilage injury were 57.89%(11/19),97.53%(79/81),and 90.00%(90/100).Conclusion MRI diagnosis of knee joint injury in children can obtain good sensitivity,specificity and accuracy,which can provide valuable reference for clinical diagnosis and treatment of knee joint injury,it is also a safe and effective diagnostic method for knee joint injury.However,it should be noted that for children with mild knee joint injury,the sensitivity of MRI is low,so clinical attention should be paid to this aspect.
