摘要
目的通过分析探讨临床药师参与的1例肾移植术后肺部感染患者的抗菌药物选择情况及药学监护,为临床合理使用抗菌药物提供参考。方法临床药师参与1例因慢性肾功能不全尿毒症期行肾移植术后肺部感染的会诊,根据患者病理生理情况、辅助检查、抗菌药物抗菌谱以及药物相互作用等特点提出抗菌药物治疗方案。结果医师采纳了临床药师提出的治疗以及用药监护建议,通过治疗患者体温、感染指标以及血培养结果均恢复正常。结论临床药师可以从药学专业知识的角度协助医师给予患者个体化治疗方案的优化,同时减少药物的不良反应。
Objective In order to provide reference for rational use of antibiotics inclinic,the selection and pharmaceutical care of antibiotics by clinical pharmacists in a patient with pulmonary infection after kidney transplantation were analyzed and discussed.Methods Clinical pharmacists participated in the consultation of a case of pulmonary infection after kidney transplantation due to chronic renal inefficacy in the stage of uremia.According to the characteristics of the patient’s pathophysiology,auxiliary examination,antimicrobial spectrum and drug interaction,the antimicrobial therapy plan was proposed.Results The physicians adopted the suggestions of the clinical pharmacists for treatment and medication monitoring,and the patient’s temperature,infection indexes and blood culture results all returned to normal after treatment.Conclusion From the perspective of pharmaceutical expertise,clinical pharmacists can assist physicians to optimize individualized treatment plans for patients and reduce adverse drug reactions.
