清醒期儿童闭目无枕区优势节律的探究

摘要 目的探究不同病因儿童清醒期闭目状态未出现枕区优势节律(α节律)的原因。方法选取2017年1月至2020年1月于广西壮族自治区人民医院门诊就诊或在儿科住院部治疗并行视频脑电图(VEEG)检查且未出现α节律的42例儿童作为研究对象,回顾性分析其临床资料及VEEG资料。结果42例儿童在清醒闭目状态时均未出现α节律,其中35例为低波幅去同步化(83.3%);4例以弥漫性低-中波幅θ波为主,夹杂少量α活动(9.5%);3例为不规则θ活动混合β波(7.1%)。病种涉及:急性淋巴细胞性白血病伴或不伴中枢神经系统白血病12例(28.6%)、儿童抽动障碍7例(16.7%)、注意力缺陷多动障碍6例(14.3%)、热性惊厥4例(9.5%)、系统性红斑狼疮3例(7.1%)、过敏性紫癜合并紫癜性肾炎3例(7.1%)、抑郁症(轻-中度)2例(4.8%)、偏头痛2例(4.8%)、头晕呕吐待查2例(4.8%)、语言发育迟缓1例(2.4%)。结论5岁以上儿童清醒闭目状态时不出现α节律应属非特异性异常,但有可能更倾向于儿童认知功能障碍类疾病。另外,长期口服激素有可能导致α指数明显下降,影响脑电背景活动。 Objective To explore the reasons for the absence of dominant occipital rhythm(αrhythm)in awake closed eyes of children with different etiologies.Methods From January 2017 to January 2020,42 children who underwent video electroencephalogram(VEEG)and didn’t haveαrhythm in the Outpatient Department or Department of Pediatrics of People’s Hospital of Guangxi Zhuang Autonomous Region were selected as the research objects,and their clinical data and VEEG data were analyzed retrospectively.Results None of the 42 children hadαrhythm in awake closed eyes,of whom 35 cases had low-amplitude desynchronization(83.3%),4 cases had diffuse low-medium amplitude wave with littleαactivity(9.5%),and 3 cases had irregularθactivity mixed withβwaves(7.1%).The diseases involved acute lymphocytic leukemia with or without central nervous system leukemia in 12 cases(28.6%),tic disorder in 7 cases(16.7%),attention deficit hyperactivity disorder in 6 cases(14.3%),febrile convulsion in 4 cases(9.5%),systemic lupus erythematosus in 3 cases(7.1%),allergic purpura combined with purpuric nephritis in 3 cases(7.1%),depression(mild to moderate)in 2 cases(4.8%),migraine in 2 cases(4.8%),dizziness and vomiting to be investigated in 2 cases(4.8%),language retardation in 1 case(2.4%).Conclusion The absence ofαrhythm in awake closed eyes in children over 5 years old should be a nonspecific anomaly,but may be more likely to be associated with cognitive dysfunction in children.In addition,long-term oral corticosteroids may lead to a significant decline in indexαand affect EEG background activity.
