摘要
目的 探究外固定支架联合加压钢板内固定序贯治疗胫腓骨开放性骨折的临床疗效:方法 选取2017年1月至2020年7月舞阳县保和卫生院收治的58例胫腓骨开放性骨折患者作为研究对象,按照治疗方法将其分为序贯治疗组(31例)与终末治疗组(27例),序贯治疗组患者早期行外固定支架治疗后更换为加压钢板内固定治疗,终末治疗组患者单纯行外固定支架治疗,对比两组患者下肢功能恢复情况、携带外固定支架时间、骨折愈合时间、住院时间以及并发症发生情况。结果 治疗12个月后,序贯治疗组患者下肢功能优良率为96.77%,明显优于终末治疗组患者的下肢功能优良率77.78%(χ^(2)=4.907,P=0.027);序贯治疗组患者携带外固定支架时间、骨折愈合时间及住院时间均明显短于终末治疗组(t=51.240、3.578、2.522,P<0.001、P<0.001、P=0.015);序贯治疗组患者术后并发症发生率为3.23%,终末治疗组患者术后并发症发生率为11.11%,两组固无明显差异(χ^(2)=1.397,P=0.237)。结论 与单纯应用外固定支架治疗相比,外固定支架拆除后更换为加压钢板内固定的序贯治疗方式,可明显提高胫腓骨开放性骨折患者下肢功能恢复效果,缩短骨折愈合时间及住院时间,疗效更显著。
Objective To study the clinical efficacy of the sequential application of external fixator combined with compression plate internal fixation in the treatment of open tibiofibular fracture.Methods 58 patients with open tibiofibular fracture,admitted into Wuyang Baohe Health Center from January 2017 to July 2020 were enrolled as research subjects to be divided into sequential treatment group(n=31) and end-stage treatment group(n=27).Patients in sequential treatment group were treated with external fixator at the early stage and then switched to receive compression plate internal fixation,while patients in end-stage treatment group were simply treated with external fixator.The functional rehabilitation of lower extremities,time of carrying external fixator,fracture healing time,length of stay and occurrence of complications were compared between the two groups.Results After 12 months of treatment,the excellent and good rate in sequential treatment group was 96.77%,obviously better than 77.78% in end-stage treatment group(χ^(2)=4.907,P=0.027).The time of carrying external fixator,fracture healing time and length of stay in sequential treatment group were all markedly shorter than end-stage treatment group(t=51.240,3.578 and 2.522,P <0.001,P <0.001,P=0.015).The incidence of postoperative complications in sequential treatment group was 3.23%,and that in end-stage treatment group was 11.11%,between which no significant difference was observed(χ^(2)=1.397,P=0.237).Conclusion Compared with the application of external fixator,the sequential application of compression plate internal fixation after the removal of external fixator can significantly improve the functional rehabilitation of lower extremities of patients with open tibiofibular fracture,and shorten their fracture healing time and length of stay,and the clinical efficacy was more definite.
