摘要
目的探讨标准化护理管理在预防关节置换术后下肢深静脉血栓形成(deep vein thrombosis,DVT)中的护理作用。方法选择2018年8月—2021年1月于我院行关节置换术患者158例作为研究对象,按照随机数字表法分为两组,每组各79例。对照组采用常规护理,观察组在对照组基础上联合标准化护理管理。护理3个月后,对患者护理效果进行评估,比较两组DVT发生率、依从性、凝血功能、下肢周径变化。结果观察组护理1个月后DVT发生率低于对照组(P<0.05);观察组护理3个月后康复锻炼、DVT预防及饮食干预依从性高于照组(P<0.05);两组护理3个月后凝血功能均得到明显改善,观察组护理3个月后凝血功能PT、TT时间长于对照组(P<0.05),APTT时间及D-D水平均短于对照组(P<0.05);两组护理3个月后下肢周径均出现不同程度增加,观察组护理3个月后患肢踝上5 cm、髌下10cm及髌上15 cm周径均小于对照组(P<0.05)。结论标准化护理管理用于关节置换术后患者中能预防并降低DVT发生率,有助于改善患者凝血功能,减少下肢周径变化幅度,可提高患者护理依从性,值得推广应用。
Objective To explore the effect of standardized nursing management in preventing deep vein thrombosis(DVT)of lower limbs after joint replacement.Methods A total of 158 patients undergoing joint replacement in our hospital from August 2018 to January 2021were selected as research object.According to random number table method,they were divided into two groups,79 cases in each group.The control group was treated with routine nursing care,and the observation group was combined with standardized nursing management on the basis of the control group.After 3 months of nursing,the nursing effect of patients was evaluated,and the incidence of DVT,compliance,coagulation function and lower limb circumference changes of the two groups were compared.Results The incidence of DVT in the observation group was lower than that of the control group for one month of nursing care(P<0.05);the compliance of rehabilitation exercise,DVT prevention and diet intervention in the observation group were higher than those of the control group after 3 months of nursing(P<0.05).The coagulation function of the two groups was significantly improved after 3 months of nursing.The PT and TT time of the observation group were longer than those of the control group(P<0.05),and the APTT time and D-D level were shorter than those of the control group(P<0.05).The lower limb circumference of the two groups increased to varying degrees after 3 months of nursing,and the circumference diameter of 5 cm above the ankle,10 cm below the patellar and 15 cm above the patellar of the affected limb in the observation group was smaller than that in the control group(P<0.05).Conclusion Standardized nursing management for patients after joint replacement can prevent and reduce the incidence of DVT,help to improve patients’coagulation function,reduce the range of changes in lower limb circumference,and improve patients’nursing compliance,which is worthy of promotion and application.
