摘要
目的探讨2型糖尿病(T2DM)患者血清正常范围的甲状腺激素水平对肌少症相关指标的影响。方法纳入2020年4~11月在安徽医科大学第一附属医院就诊的192例40~70岁且甲状腺激素水平处于正常范围的T2DM患者(男105例,女87例),收集患者一般资料、血糖、血脂、甲状腺功能,采用生物电阻测量法(BIA)测定四肢骨骼肌含量、香山EH101测量握力、简易体能状况量表评分(SPPB)测量躯体功能,并分析促甲状腺激素(TSH)、总T4(TT4)、总T3(TT3)、游离甲状腺激素(FT4)、游离三碘甲腺原氨酸(FT3)与肌少症相关指标的关系。结果研究对象中肌少症检出率为6.25%,年龄与四肢骨骼肌质量(ASM)、四肢骨骼肌质量指数(ASMI)、SPPB呈负相关,性别与ASM、ASMI、握力呈正相关,TT3与SPPB评分呈负相关(β=-0.324,P=0.047),TT4与ASMI、ASM呈负相关,TSH与握力呈负相关(β=-0.133,P=0.032)。结论在T2DM患者中,TT3、TT4、TSH与肌肉相关指标有关,临床上应关注T2DM患者的甲状腺功能状态,有助于早期识别和筛查肌少症高危个体。
Objective To explore the influence of serum thyroid hormone levels in the normal range on the related indicators of sarcopenia in patients with type 2 diabetes mellitus(T2DM).Methods A total of 192 T2DM patients(105 males and 87 females)with normal thyroid hormone levels between 40 and 70 years old in the First Affiliated Hospital of Anhui Medical University from April 2020 to November 2020 were enrolled.General data of the patients were collected.Blood glucose,blood lipids,thyroid function,bioelectrical impedance analysis(BIA)was used to measure appendicular skeletal muscle,Xiangshan EH101 to measure handgrip strength,SPPB to measure physical performance,and to analyze the relationship between thyroid stimulating hormone(TSH),total thyroxine(TT4),total triiodothyronine(TT3),free thyroxine(FT4),free triiodothyronine(FT3)and related indicators of sarcopenia.Results The prevalence rate of sarcopenia was 6.25%.Age was negatively correlated with appendicular skeletal muscle mass(ASM),appendicular skeletal muscle mass index(ASMI),and the score of short physical performance battery(SPPB).Gender was negatively correlated with ASM and ASMI.Handgrip strength was positively correlated,TT3 was negatively correlated with the score of SPPB(β=-0.324,P=0.047).TT4 was negatively correlated with ASMI and ASM,where as TSH was negatively correlated with handgrip strength(β=-0.133,P=0.032).Conclusion In patients with T2DM,TT3,TT4,and TSH are related to muscle-related indicators.Clinical attention should be paid to the thyroid function status of patients with T2DM,which is helpful for early identification and screening of high-risk individuals.
