摘要
目的 观察小青龙汤加减联合痉咳静治疗小儿过敏性咳嗽(allergic cough,AC)风寒型的临床疗效。方法 选取2017年10月至2019年10月在南京中医药大学附属盐城市中医院儿科门诊、急诊就诊的AC风寒型患儿60例,采用随机数字表法分为对照组和治疗组,各30例。对照组予地氯雷他定糖浆口服治疗,治疗组采用小青龙汤加减联合痉咳静口服治疗,1周为1个疗程,2组均连续治疗2个疗程后统计临床疗效。结果 治疗组的总有效率为86.7%(26/30),显著高于对照组的63.3%(19/30),2组比较,差异有统计学意义(P<0.05)。治疗组治愈患儿咳嗽、咳痰、咽痒、鼻塞流涕、多汗、睡时打鼾等症状消失时间明显短于对照组,2组比较,差异有统计学意义(P<0.05);而鼻痒和纳差消失时间与对照组比较,差异无统计学意义(P>0.05)。治疗前,2组的临床症状评分比较,差异无统计学意义(P>0.05),具有可比性;治疗后2组咳嗽、咳痰、鼻塞流涕、鼻痒、咽痒等临床症状评分均较同组治疗前明显降低,差异均有统计学意义(P<0.05),且治疗组的咳嗽、咳痰、鼻塞流涕、咽痒等临床症状评分下降更显著(P<0.05),仅鼻痒临床症状评分与对照组治疗后比较,差异无统计学意义(P>0.05)。结论 小青龙汤加减联合痉咳静治疗小儿AC风寒型,疗效确切,可显著缓解临床症状,明显缩短治愈患儿的临床症状消失时间,值得临床推广应用。
Objective To observe the clinical efficacy of modified Xiaoqinglong Tang(小青龙汤)combined with Jingkejing(痉咳静)in the treatment of allergic cough(AC)in children with wind-cold type.Methods From October 2017 to October 2019,60 cases of AC in children with wind-cold type consulted in outpatient service and emergency clinic of pediatrics department of Yancheng Hospital of TCM Affiliated to Nanjing University of Chinese Medicine were chosen and divided into control group and treatment group according to random number table method,with 30 cases in each group.The control group was given oral administration of desloratadine syrup,while the treatment group was given oral administration of modified Xiaoqinglong Tang combined with Jingkejing.One week constituted 1 course of treatment,and we kept statistics about the clinical efficacy after the 2 groups were both continuously treated for 2 courses of treatment.Results The total effective rate of the treatment group was 86.7%(26/30),which was significantly higher than 63.3%(19/30)of the control group,and the difference was statistically significant between the 2 groups(P<0.05).The disappearance time of symptoms such as cough,expectoration,throat tickle,stuffy and runny nose,hyperhidrosis and snoring during sleep of cured cases in the treatment group was significantly shorter than that in the control group,and the difference was statistically significant between the 2 groups(P<0.05);There was no statistically significant difference in the disappearance time of nasal itching and poor appetite than that in the control group(P>0.05).Before treatment,there was no statistically significant difference in clinical symptom score between the 2 groups(P>0.05),indicating comparability.After treatment,the scores of clinical symptoms,namely cough,expectoration,stuffy and runny nose,nasal itching and throat tickle in the 2 groups were significantly lower than those in the same group before treatment,and the difference was statistically significant(P<0.05),moreover,the scores of clinical symptoms,namely cough,expectoration,stuffy and runny nose and throat tickle etc.in the treatment group decreased more significantly(P<0.05).There was no statistically significant difference in clinical symptom score of nasal itching compared with that in the control group after treatment(P>0.05).Conclusion It has definite efficacy to use modified Xiaoqinglong Tang combined with Jingkejing in the treatment of AC with wind-cold type in children,which can markedly alleviate clinical symptoms and shorten the disappearance time of clinical symptoms of the cured cases.It is worthy of clinical promotion and application.
