立体定向颅内血肿清除术对急性脑出血患者神经功能、炎症因子及外周血miR-21的影响 被引量:2

摘要 目的:探讨在急性脑出血(ICH)患者中应用立体定向颅内血肿清除术治疗对其神经功能、炎症因子及外周血miR-21的影响。方法:选取2018年6月-2020年6月在景德镇市第一人民医院接受治疗的82例急性ICH患者作为研究对象,根据随机数字表法将患者分为观察组(n=41)与对照组(n=41)。对照组实行小骨窗血肿清除术治疗,观察组实行立体定向颅内血肿清除术治疗。比较两组神经功能、炎症因子、神经损伤及外周血miR-21。结果:治疗后,两组美国国立卫生研究院卒中量表(NIHSS)评分、神经元特异性烯醇化酶(NSE)、胶质纤维酸性蛋白(GFAP)、神经功能性因子(S100B)、miR-21水平均低于治疗前,且观察组均低于对照组,差异均有统计学意义(P<0.05)。治疗后,两组白介素-10(IL-10)水平均高于治疗前,白介素-18(IL-18)、肿瘤坏死因子-α(TNF-α)水平均低于治疗前,且观察组均优于对照组,差异均有统计学意义(P<0.05)。结论:立体定向颅内血肿清除术应用于急性ICH患者中可促进神经功能与炎症因子的改善,缓解神经损伤,降低外周血miR-21水平。 Objective:To investigate the effect of stereotactic intracranial hematoma removal on nerve function,inflammatory factors and peripheral blood miR-21 in patients with acute cerebral hemorrhage (ICH).Method:A total of 82 patients with acute ICH who treated in the Jingdezhen First People’s Hospital from June 2018 to June 2020 were selected as the research objects,and the patients were divided into the observation group (n=41) and the control group (n=41) according to the random number table method.The control group was treated with small bone window hematoma removal,and the observation group was treated with stereotactic intracranial hematoma removal.The nerve function,inflammatory factors,nerve damage and peripheral blood miR-21 were compared between two groups.Result:After treatment,the national institutes of health stroke scale (NIHSS) score,neuron-specific enolase (NSE),glial fibrillary acidic protein (GFAP),neurofunctional factor (S100B),and miR-21 levels in two groups were lower than those before treatment,and those in the observation group were lower than those in the control group,the differences were statistically significant (P<0.05).After treatment,the levels of interleukin-10 (IL-10) in two groups were higher than those before treatment,and the levels of interleukin-18 (IL-18) and tumor necrosis factor-α (TNF-α) were lower than those before treatment,and the observation group were better than those in the control group,the differences were statistically significant (P<0.05).Conclusion:The application of stereotactic intracranial hematoma removal in patients with acute ICH can promote the improvement of nerve function and inflammatory factors,relieve nerve damage,and reduce peripheral blood miR-21.
