围绝经期女性献血者铁营养状况及血红蛋白水平调查分析

摘要 目的:调查分析围绝经期女性全血献血者血浆铁蛋白(Ft)以及血红蛋白(Hb)水平。方法:选取九江市中心血站2021年1-12月120例围绝经期女性全血献血者作为研究对象,根据累计献血次数将献血者分成对照组(初次献血者即累计献血次数为1次,n=68)和研究组(固定献血者即累计献血次数≥3次,n=52)。将研究组按照献血次数分为累计献血3次组(n=22)、累计献血4次组(n=18)、累计献血5次组(n=12)。检测并比较各组献血者的红细胞计数(RBC)、红细胞比容(HCT)、血红蛋白(Hb)、血浆铁(PI)、血浆铁蛋白(Ft)水平。结果:对照组和研究组的RBC比较,差异无统计学意义(P>0.05);研究组的HCT、Hb均低于对照组,差异均有统计学意义(P<0.05)。研究组的PI、Ft均低于对照组,差异均有统计学意义(P<0.05)。三组的RBC比较,差异无统计学意义(P>0.05);累计献血5次组HCT<累计献血4次组HCT<累计献血3次组HCT,累计献血5次组Hb<累计献血4次组Hb和累计献血3次组Hb,差异均有统计学意义(P<0.05)。累计献血5次组Ft<累计献血4次组Ft<累计献血3次组Ft,累计献血5次组PI<累计献血4次组PI和累计献血3次组PI,差异均有统计学意义(P<0.05)。HCT、Hb、PI、Ft均与献血次数呈负相关(P<0.05)。结论:多次献血的围绝经期女性全血献血者铁营养状况和血红蛋白水平低于初次献血的围绝经期女性全血献血者。且随着献血次数的增加,其铁营养状况和血红蛋白水平均下降,但仍在标准值的范围内,因此需要对长期多次献血的围绝经期女性全血献血者进行铁营养状况的跟踪和健康指导。 Objective:To investigate and analyze the plasma ferritin(Ft)and hemoglobin(Hb)levels of perimenopausal female whole blood donors.Method:A total of 120 perimenopausal female whole blood donors from Jiujiang Central Blood Station from January to December 2021 were selected as the research objects,according to the accumulated times of blood donation,the blood donors were divided into control group(the first blood donor had donated blood once,n=68)and study group(fixed blood donors,accumulated times of blood donation≥3 times,n=52).According to the times of blood donation,the study group was divided into 3 times accumulative blood donation group(n=22),4 times accumulative blood donation group(n=18)and 5 times accumulative blood donation group(n=12).The levels of red blood cell count(RBC),red blood cell volume(HCT),hemoglobin(Hb),plasma iron(PI)and plasma ferritin(Ft)were detected and compared.Result:There was no significant difference in RBC between control group and study group(P>0.05).The HCT and Hb of the study group were lower than those of the control group,the differences were statistically significant(P<0.05).The PI and Ft of the study group were lower than those of the control group,the differences were statistically significant(P<0.05).There was no significant difference in RBC between the three groups(P>0.05);HCT of accumulative blood donation 5 times group<accumulative blood donation 4 times group<accumulative blood donation 3 times group,Hb of accumulative blood donation 5 times group<accumulative blood donation 4 times and accumulated blood donation for 3 times,the differences were statistically significant(P<0.05).Ft of accumulative blood donation 5 times group<accumulative blood donation 4 times group<accumulative blood donation 3 times group,PI of accumulative blood donation 5 times group<accumulative blood donation 4 times and accumulated blood donation for 3 times,the differences were statistically significant(P<0.05).HCT,Hb,PI and Ft were negatively correlated with the times of blood donation(P<0.05).Conclusion:The iron nutritional status and hemoglobin level of perimenopausal female whole blood donors who donate blood for many times are lower than that of perimenopausal female whole blood donors who donate blood for the first time.With the increase of blood donation times,their iron nutritional status and hemoglobin level decrease,but they are still within the range of standard values,therefore,it is necessary to track the iron nutritional status and health guidance for perimenopausal female whole blood donors who donate blood for many times for a long time.
