摘要
目的探究踝关节骨折术后短腿石膏临时外固定在预防足下垂中的应用效果。方法选取2018年6月—2020年7月于大余县人民医院行踝关节骨折手术的患者50例,采用随机数字表法分为A组及B组,各25例。A组采用短腿石膏临时外固定,B组采用简易绷带包扎,2组均持续观察至术后6个月,比较2组踝关节功能优良率、踝关节活动度、踝关节功能评分及足下垂发生率。结果A组踝关节功能优良率为88.00%,高于B组的64.00%,差异有统计学意义(P<0.05)。术后2 d,2组踝关节活动度比较,差异无统计学意义(P>0.05);术后3周、术后3个月、术后6个月,A组踝关节活动度均优于B组,差异有统计学意义(P<0.05)。A组术后3个月及术后6个月踝关节功能评分均高于B组,差异有统计学意义(P<0.05)。A组足下垂发生率为0,低于B组的24.00%,差异有统计学意义(P<0.05)。结论踝关节骨折术后采用短腿石膏临时外固定能够有效恢复踝关节功能及活动度,并可降低足下垂发生率,疗效确切。
Objective To explore the effect of short leg plaster temporary external fixation in the prevention of foot ptosis after ankle fracture surgery.Methods 50 patients who underwent ankle fracture surgery in Dayu County People's Hospital from June 2018 to July 2020 were randomly divided into group A and group B,with 25 cases in each group.Group A was temporarily fixed with short leg plaster,group B was only wrapped with simple bandage,and both groups were observed until 6 months after operation.The excellent and good rate of ankle function,ankle range of motion,ankle function score and the incidence of foot sag were compared between the two groups.Results The excellent and good rate of ankle function in group A was 88.00%,which was higher than 64.00%in group B(P<0.05);There was no significant difference in ankle range of motion between the two groups 2 days after operation(P>0.05);At 3 weeks,3 months and 6 months after operation,the ankle range of motion in group A was better than that in group B(P<0.05);The ankle function scores in group A were higher than those in group B at 3 and 6 months after operation(P<0.05);The incidence of foot ptosis in group A was 0,which was lower than 24.00%in group B(P<0.05).Conclusion Short leg plaster temporary external fixation after ankle fracture surgery can effectively restore ankle function and range of motion,and reduce the incidence of foot droop.
