摘要
目的探讨阿霉素联合他巴唑诱导慢性肾脏病肾阳虚证大鼠模型的建立方法与评价及真武汤干预作用的研究。方法SPF级体质量200-220 g的SD雄性大鼠52只,适应性喂养2周,随机分为正常组10只、模型组42只,模型组大鼠第3周、第4周分别按照3 mg·kg^(-1)、4mg·kg^(-1)体重尾静脉注射阿霉素进行慢性肾脏病大鼠模型造模。模型成功大鼠按体质量与尿蛋白进行分层随机,分为阿霉素组14只、他巴唑组14只、真武汤组14只,第13周起真武汤组按照5.39 g/kg/天进行干预。观察大鼠一般情况,每周称取大鼠体质量,每4周检测大鼠24 h尿蛋白定量、采用红外测温枪检测大鼠体温,采用大鼠游泳实验检测游泳时间。24周后股动脉取血并处死大鼠,称取大鼠肾脏、脾脏、睾丸重量并计算脏器指数,采用HE染色、Masson染色观察各组大鼠肾组织病理情况。采用比色法检测血清Scr、BUN、UA、TC、TG、TP、ALB、SOD的水平;采用酶联免疫吸附法检测血清T3、T4、TSH、FSH、LH、T、E2、CORT的水平。结果与正常组比较,他巴唑组大鼠出现毛发枯槁、弓背、懒动、蜷缩、喜扎堆、肛周污秽、体型较为瘦弱、行动迟缓等典型肾阳虚体征,真武汤组较他巴唑组一般情况有所好转,肾阳虚体征减轻,体重显著升高(P<0.01),肾脏指数、24 h尿蛋白定量明显下降(P<0.01),体温、游泳时间增加;肾脏组织HE、Masson染色结果显示,阿霉素组、他巴唑组肾小球系膜区增宽,系膜细胞增生,系膜区有大量胶原纤维沉积,肾小管扩张或萎缩,存在蛋白管型,肾间质有大量炎性细胞浸润,阿霉素组球囊壁粘连明显,真武汤组病理变化有所改善;血清指标显示,与他巴唑组相比,真武汤组Scr、UA、BUN、TC、TG有降低趋势,ALB含量升高(P<0.05);甲状腺轴指标T3有升高趋势,T4显著升高(P<0.01),TSH显著降低(P<0.01);真武汤组性腺轴指标FSH、LH、E2显著升高(P<0.01),T无差异;肾上腺轴指标CORT各组之间无统计学差异;真武汤组SOD显著升高(P<0.01)。结论阿霉素联合他巴唑可以成功建立慢性肾脏病肾阳虚证动物模型,真武汤干预作用也证明了该病证结合模型的成功建立,该模型主要模拟了慢性肾脏病下丘脑-垂体-甲状腺轴功能低下的肾阳虚证动物模型。
Objective To explore the establishment method and evaluation of adriamycin combined with Thiamazole inrats with kidney yang deficiency syndrome of chronic kidney disease and the intervention effect of Zhenwu Decoction.Methods 52 male Sprague Dawley rats with SPF weight of 200-220 g were randomly divided into normal group(n=10)and model group(n=42)after 2 weeks of adaptive feeding.The rats in model group were injected with adriamycin viatail vein at the weight of 3 mg·kg^(-1)and 4 mg·kg^(-1) respectively at the 3;and 4;week to establish the rat model of chronickidney disease.The rats with successful model were randomly divided into adriamycin group(n=14),Thiamazole group(n=14)and Zhenwu Decoction group(n=14),according to body mass and urinary protein.From the 13;week,Zhenwu Decoction group was intervened according to 5.39 g/kg/d.We observed the general situation of rats,weighed the bodyweight of rats every week,measured the 24 h urinary protein of rats every 4 weeks,detected the body temperature of ratswith infrared thermometer,and detected the swimming time with rat swimming experiment.After 24 weeks,blood wastaken from the femoral artery and the rats were killed.The weight of kidney,spleen and testis were measured and theorgan index was calculated.HE staining and Masson staining were used to observe the pathological changes of renaltissue.The levels of serum Scr,BUN,UA,TC,TG,TP,ALB and SOD were detected by colorimetry;The levels of serumT3,T4,TSH,FSH,LH,T,E2,CORT were detected by ELISA.Results Compared with the normal group,the rats in theThiamazole group showed typical signs of kidney yang deficiency,such as haggard hair,arched back,lazy movement,curling up,like to pile up,perianal filth,thin body,slow action,etc.The general situation in the Zhenwu Decoctiongroup was better than that in the Thiamazole group.The signs of kidney yang deficiency were lighter,and the bodyweight was significantly increased(P<0.01).The kidney index and 24-hour urine protein were significantly decreased(P<0.01).Body temperature and swimming time increased.The results of HE and Masson staining showed that theglomerular mesangial area widened,mesangial cells proliferated,a large number of collagen fibers deposited inmesangial area,renal tubules dilated or atrophied,protein tubules existed,and a large number of inflammatory cellsinfiltrated in renal interstitium in adriamycin group and Thiamazole group.The adhesion of balloon wall was obvious inadriamycin group,and the pathological changes were improved in Zhenwu Decoction group.Compared with theThiamazole group,the levels of Scr,UA,BUN,TC and TG in the Zhenwu Decoction group decreased,while the levels ofALB increased(P<0.05);Thyroid axis index T3 increased,T4 increased significantly(P<0.01),TSH decreasedsignificantly(P<0.01).In the Zhenwu Decoction group,FSH,LH and E2 were significantly increased(P<0.01),but Thad no difference.There was no significant difference in CORT;The SOD of Zhenwu Decoction group increasedsignificantly(P<0.01).Conclusion Adriamycin combined with Thiamazole can successfully replicate the animal modelof chronic kidney disease with kidney-yang deficiency syndrome.The intervention of Zhenwu Decoction also proves thesuccessful establishment of the combined disease and syndrome model,which mainly simulates the hypothalamus-pituitary-thyroid gland of chronic kidney disease.An animal model of kidney-yang deficiency syndrome with low axialfunction.
