(18)^F-氟代脱氧葡萄糖正电子发射计算机体层显像对原发性肺癌术前纵隔淋巴结N分期的诊断价值 被引量:3

摘要 目的探讨(18)^F-氟代脱氧葡萄糖正电子发射计算机体层显像((18)^F-FDGPET/CT)对原发性肺癌术前纵隔淋巴结N分期的诊断价值。方法收集经病理检查确诊的92例原发性肺癌患者的病历资料,所有患者术前均进行(18)^F-FDGPET/CT和CT检查。以病理结果为金标准,比较(18)^F-FDGPET/CT和CT诊断原发性肺癌的符合率,并分析PET/CT诊断原发性肺癌术前纵隔淋巴结N分期的准确率。结果92例原发性肺癌中,经CT检出78例,诊断符合率为84.78%;经PET/CT检出87例,诊断符合率为94.57%。PET/CT诊断肺癌的符合率高于CT(P﹤0.05)。PET/CT对原发性肺癌N_(0)期的诊断准确率为96.97%,对N_(1)期的诊断准确率为90.00%,对N_(2)期的诊断准确率为92.86%,对N_(3)期的诊断准确率为90.91%,对N分期的诊断准确率为93.48%。结论(18)^F-FDGPET/CT对原发性肺癌具有较高的诊断价值,且对术前纵隔淋巴结N分期的诊断准确率较高,可为临床制订合理的治疗方法提供可靠依据。 Objective To investigate the diagnostic value of (18)^F-fluorodeoxyglucose position emission tomography/computer tomography((18)^F-FDG PET/CT)in mediastinal lymph node N staging of primary lung cancer before surgery.Method The clinical data of 92 patients with primary lung cancer confirmed by pathology were collected.All patients received (18)^F-FDG PET/CT and CT examination before surgery.By using pathological results as the gold standard,the coincidence rate of (18)^F-FDG PET/CT and CT in diagnosing primary lung cancer was compared,and the accuracy of PET/CT in evaluating preoperative N staging of the mediastinal lymph nodes of primary lung cancer was analyzed.Result A total of 78 and 87 cases in 92 cases with primary lung cancer were detected by CT and PET/CT,respectively,and the diagnostic coincidence rate of PET/CT was higher than that of CT(94.57%vs 84.78%,P<0.05).The diagnostic accuracy of PET/CT for N_(0) stage was 96.98%,N_(1) stage 90.00%,N_(2) stage 92.86%,and N_(3) stage 90.91%.For N staging,the overall diagnostic accuracy was 93.48%.Conclusion (18)^F-FDG PET/CT has higher diagnostic value in primary lung cancer and higher diagnostic accuracy for preoperative mediastinal lymph node N staging,which can be used to provide reliable evidence for reasonable treatment.
