摘要
目的:探究基于膀胱功能分类评估的康复护理应用于神经源性膀胱患儿的效果。方法:选取深圳市儿童医院2018年12月-2021年6月纳入的172例神经源性膀胱患儿,按照简单随机数字表法分为对照组(n=86)和观察组(n=86),对照组采用常规护理干预,观察组在对照组的基础上采用基于膀胱功能分类评估的康复护理。评估两组患儿的临床疗效、尿动力学指标、尿路感染情况、生命质量评分情况。结果:干预后,观察组总有效率为94.00%,高于对照组的84.00%(P<0.05);观察组患儿的尿动力学指标中的膀胱容量和膀胱顺应性均高于对照组,观察组患儿充盈末逼尿肌压低于对照组(P<0.05);观察组患儿尿路感染率低于对照组(P<0.05);与干预前相比,干预后两组患儿生命质量各项目评分均明显降低,且观察组低于对照组(P<0.05)。干预后,观察组患儿生命质量各项目评分均低于对照组(P<0.05)。结论:基于膀胱功能分类评估的康复护理应用于神经源性膀胱患儿具有良好的临床效果,能有效改善患儿尿动力学指标,减少尿路感染情况,提高患儿的生命质量。
Objective:To explore the effect of rehabilitation care applied to children with neurogenic bladders based on categorical assessment of bladder function.Method:A total of 172 children with neurogenic bladder enrolled from December 2018 to June 2021 in Shenzhen Children’s Hospital were divided into control group(n=86)and observation group(n=86)according to the simple random number table method,the control group received routine nursing intervention,and the observation group received rehabilitation care based on the categorical assessment of bladder function on the basis of the control group.Clinical outcomes,urodynamic parameters,urinary tract infections,and quality of life scores were assessed in two groups.Result:After intervention,the total effect rate of the observation group was 94.00%,which was higher than 84.00%of the control group(P<0.05).The bladder capacity and bladder compliance of the urodynamic indicators in the observation group were higher than those in the control group,and children in the observation group had lower detrusor pressure at the end of filling than that in the control group(P<0.05).Rate of urinary tract infections in the observation group was lower than that in the control group(P<0.05).Compared with before intervention,the scores of each item of child’s quality of life in the two groups after intervention were significantly lower,and those were lower in the observation group than those in the control group(P<0.05).Children in the observation group had lower scores in all items of quality of life than those in the control group(P<0.05).Conclusion:Rehabilitation care based on the classification assessment of bladder function applied to children with neurogenic bladder has good clinical effects,can effectively improve the children’s urodynamic indicators,reduce urinary tract infection,and improve the children’s quality of life.
