摘要
目的:分析新冠疫情发生前后某地区多重耐药菌(MDR)的临床分布及流行状况,为医院在疫情常态化下,进一步加强MDR感染的预防、控制工作提供理论依据。方法:分别收集2019年1月~2020年12月来自某地区的9家医疗单位8家三级医院和1家二级医院的各类送检标本,按照统一方案进行细菌培养鉴定及耐药性检测,采用质谱方法进行细菌鉴定,采用纸片扩散法(K-B法)、自动化仪器法进行细菌的耐药性检测,参照美国临床实验室标准化协会(CLSD)2019年版标准进行结果判定,用Whone6.6软件时数据进行统计分析。结果:2020年共检出MDR 2052株,总检出率为16.25%,低于2019年(2455株)总检出率18.2%。2020年新冠疫情爆发时期CR-KPN、CR-PA检出率均低于与2019年的检出率;而2020年VRE高于2019年检出率。2020年重症医学科和呼吸内科的多重耐药菌株检出率与疫情前(2019年)相比呈上升态势,差异有统计学意义(P<0.05);而心内科、感染疾病科、消化内科则降低。结论:在新冠疫情期间,更应加强相关科室MDR的监测及MDR患者的隔离措施,防止院内感染的发生。
Objective:To analyze the clinical distribution and epidemic situation of multi-drug-resistant bacteria(MDR)in a certain area before and after the outbreak of COVID-19,so as to provide theoretical basis for hospitals to further strengthen the prevention and control of MDR infection in the present epidemic situation.Methods:Samples from 9 medical units in the region during January 2019 to December 2020,including 8 tertiary hospitals and one second-class hospital,were collected for bacterial culture identification and drug resistance detection according to the unified scheme.The bacteria were identified by mass spectrometry,and the drug resistance of bacteria was detected by Kirby-Bauer testing KB method and automatic instrument method The results were interpreted according to the 2019 edition of the American Association for Clinical Laboratory Standardization(CLSI),and the data were statistically analyzed by Whonet5.6 software.Results:A total of MDR2052 strains were detected in 2020,with a total detection rate of 16.25%,which was lower than the total detection rate of 18.2% in 2019(2455 strains).During the outbreak of COVID-19 in 2020,the detection rate of CR-KPN and CR-PA was lower than that in 2019,while the detection rate of VRE in 2020 was higher than that in 2019.The detection rate of multiple drug-resistant strains in critical medicine and respiratory medicine in 2020 was significantly higher than that before the epidemic(2019)(P<0.05),while that in cardiology,infectious diseases and gastroenterology decreased Con elusion:During the epidemic period of COVID-19,the surveillance of MDR in relevant departments and the isolation measures of MDR patients should be strengthened to prevent the occurrence of nosocomial infection.
