CHI3L1基因突变与颈动脉粥样硬化斑块稳定性的关系 被引量:2

摘要 目的检测颈动脉粥样硬化患者CHI3L1基因rs880633位点突变,对意义进行了分析。方法选取2015年1月~2019年12月确诊存在颈动脉粥样硬化斑块者502例为研究组,以未发现斑块者480例为对照。超微血管成像(SMI)技术检测斑块内新生血管分级;检测入组者血脂、Lp-PLA_(2)、YKL-40蛋白水平;检测CHI3L1基因rs880633位点突变情况。结果研究组患者血清中Lp-PLA_(2)、YKL-40、TC、LDL-C、BMI及家族史比例均明显高于对照组,而HDL-C低于对照组患者(P<0.05)。研究组患者GG及AG基因型比例、G等位基因频率明显高于对照组(P<0.01)。按SMI分级将研究组患者分为4组,4组患者的GG、AG基因型及G等位基因频率比较,差异有统计学意义(P<0.05)。不同基因型的入组者血脂、Lp-PLA_(2)、YKL-40蛋白比较,差异有统计学意义(P<0.05)。CHI3L1基因rs880633位点GG基因型与新生血管有关,是新生血管发生的独立危险因素(P<0.05)。结论CHI1L3基因rs880633位点多态性可能是导致颈动脉斑块新生血管生成的重要因素。 Objective To detect the mutation of CHI3L1 gene in patients with carotid atherosclerosis d,and explore its significanced.Methods Totally 502 patients with carotid atherosclerotic plaque from January 2015 to December 2019 were recruited as research group,and 480 cases without carotid atherosclerotic plaque as control group.Levels of neovascularization in carotid atherosclerotic plaque were assessed with SMI technique.Serum lipids,Lp-PLA_(2),YKL-40 were detected.rs880633 site polymorphism of CHI3L1 gene in patients with carotid atherosclerosis was also tested.Results The serum levels of Lp-PLA_(2),YKL-40,TC,LDL-C,BMI and positive family history were higher in research group than those of control group,while level of HDL-C was lower than that of control group(P<0.05).Ratio of GG,AG genotypes and frequency of G allele were differently distributed between research group and control group(P<0.01).The ratio of GG,AG genotypes and frequency of G allele were significantly differently according to result of MSI detection(P<0.05).There were significant differences in serum lipids,Lp-PLA_(2),YKL-40 in patients with different genotypes(P<0.05).rs880633 site GG genotype of CHI3L1 gene in patients with carotid atherosclerosis was closely associated with neovascularization in carotid atherosclerotic plaque,and it was independent risk factor of neovascularization(P<0.05).Conclusion The rs880633 polymorphism of CHI1L3 gene may be an important factor leading to carotid plaque neovascularization.
