摘要
目的:分析气压泵治疗预防内镜粘膜下剥离术(endoscopic submucosal dissection,ESD)手术患者下肢深静脉血栓形成的效果。方法:选2020年12月~2021年4月60例ESD手术患者进行研究,采用随机数表法分为观察组(30例)与对照组(30例)。对照组实施常规治疗措施进行干预,观察组实施气压泵治疗措施进行干预。对比患者D-二聚体(D-D)水平、血小板及凝血四项水平、深静脉血栓发生情况、炎性因子水平、生活质量。结果:(1)D-二聚体(D-D)水平对比:观察组患者D-D水平显著低于对照组(P<0.05),有统计学意义;(2)血小板及凝血四项水平对比:观察组患者血小板计数、纤维蛋白原显著低于对照组,凝血酶原时间、凝血活酶时间、部分凝血活酶时间显著高于对照组(P<0.05),有统计学意义;(3)深静脉血栓发生情况对比:观察组患者深静脉血栓发生率显著低于对照组(P<0.05),有统计学意义;(4)炎性因子水平对比:观察组患者TNF-α、CRP、PCT水平显著低于对照组,IFN-γ水平显著高于对照组,(P<0.05),有统计学意义;(5)生活质量对比:观察组患者生活质量显著高于对照组,(P<0.05),有统计学意义。结论:气压泵治疗可有效改善患者D-D水平,改善患者血小板及凝血四项水平,降低患者血栓发生率。
Objective:To analyze the effect of pneumatic pump therapy on the prevention of deep vein thrombosis of lower extremity in patients undergoing endoscopic submucosal dissection(ESD).Methods:From December 2020 to April 2021,60 patients with ESD surgery were selected for research,and they were divided into observation group(30 cases)and control group(30 cases)by random number table method.The control group was intervened by conventional treatment measures,and the observation group was intervened by air pressure pump therapy.The levels of D-dimer(D-D),the four levels of platelets and coagulation,the occurrence of deep vein thrombosis,the levels of inflammatory factors,and the quality of life were compared.Results:(1)D-dimer(D-D)level comparison:the D-D level in the observation group was significantly lower than that in the control group(P<0.05),with statistical significance;(2)Comparison of four levels of platelets and coagulation:the platelet count and fibrinogen in the observation group were significantly lower than those in the control group,and the prothrombin time,thromboplastin time and partial thromboplastin time were significantly higher than those in the control group(P<0.05),which is statistically significant;(3)Deep vein thrombosis occurrence comparison:the incidence of deep vein thrombosis in the observation group was significantly lower than that in the control group(P<0.05),with statistical significance;(4)Comparison of the levels of inflammatory factors:the levels of TNF-α,CRP and PCT in the observation group were significantly lower than those in the control group,and the levels of IFN-γwere significantly higher than those in the control group(P<0.05),with statistical significance;(5)Comparison of quality of life:the quality of life of patients in the observation group was significantly higher than that in the control group(P<0.05),with statistical significance.Conclusion:Air pressure pump therapy can effectively improve the D-D level of patients,improve the four levels of platelets and coagulation,and reduce the incidence of thrombosis in patients.
