乙酰半胱氨酸联合特布他林、阿奇霉素序贯疗法对小儿支气管肺炎肺功能指标及Treg、Th17、炎症因子水平的影响 被引量:12

摘要 目的探讨乙酰半胱氨酸联合特布他林、阿奇霉素序贯疗法在小儿支气管肺炎中的应用效果。方法选取2018年11月至2020年11月收治的78例支气管肺炎患儿为研究对象,采用数字表法将其分为对照组和观察组,各39例。对照组给予特布他林、阿奇霉素序贯疗法,观察组在对照组基础上给予乙酰半胱氨酸治疗。比较两组的肺功能指标、Treg、Th17、免疫球蛋白、炎症因子水平。结果治疗前,两组的FVC、FEV1及PEF比较,差异无统计学意义(P>0.05);治疗后,两组的FVC、FEV1及PEF均较治疗前升高,且观察组高于对照组(P<0.05)。治疗前,两组的Treg、Th17水平比较,差异无统计学意义(P>0.05);治疗后,两组的Treg水平均较治疗前升高,Th17水平均较治疗前降低,且观察组优于对照组(P<0.05)。治疗前,两组的IgA、IgG、IgM水平比较,差异无统计学意义(P>0.05);治疗后,两组的IgM水平均较治疗前降低,且观察组低于对照组(P<0.05)。治疗前,两组的IL-6、IL-33、TNF-α水平比较,差异无统计学意义(P>0.05);治疗后,两组的IL-6、IL-33、TNF-α水平均较治疗前降低,且观察组低于对照组(P<0.05)。结论乙酰半胱氨酸联合特布他林、阿奇霉素序贯疗法在小儿支气管肺炎中的应用效果较好,可有效改善肺功能,调节Th17/Treg平衡,减轻炎症反应。 Objective To investigate the application effect of acetylcysteine combined with terbutaline and azithromycin sequential therapy on children bronchial pneumonia.Methods A total of 78 children with bronchial pneumonia treated from November 2018 to November 2020 were selected as the research objects.The children were divided into control group and observation group by digital table method,with 39 cases in each group.The control group was given terbutaline and azithromycin sequential therapy,and the observation group was treated with acetylcysteine on the basis of the control group.The pulmonary function indexes,Treg,Th17,immunoglobulin and inflammatory factors levels were compared between the two groups.Results Before treatment,there were no significant differences in FVC,FEV1 and PEF between the two groups(P>0.05);after treatment,FVC,FEV1 and PEF in the two groups were higher than those before treatment,and those in the observation group were higher than the control group(P<0.05).Before treatment,there were no significant differences in the levels of Treg and Th17 between the two groups(P>0.05);after treatment,the level of Treg in the two groups was higher than that before treatment,the level of Th17 was lower than that before treatment,and those in the observation group were better than the control group(P<0.05).Before treatment,there were no significant differences in the levels of IgA,IgG and IgM between the two groups(P>0.05);after treatment,the level of IgM in the two groups was lower than that before treatment,and that in the observation group was lower than the control group(P<0.05).Before treatment,there were no significant differences in the levels of IL-6,IL-33 and TNF-α between the two groups(P>0.05);after treatment,the levels of IL-6,IL-33 and TNF-α in the two groups were lower than those before treatment,and those in the observation group were lower than the control group(P<0.05).Conclusion Acetylcysteine combined with terbutaline and azithromycin sequential therapy in children bronchial pneumonia has better effect.It can effectively improve pulmonary function,regulate Th17/Treg balance and reduce inflammatory response.
