摘要
目的观察右归丸对肾阳虚型多囊卵巢综合征(polycystic ovary syndrome,PCOS)不孕症患者卵泡发育及子宫内膜容受性的影响。方法将符合入选要求的60例患者随机分为2组,各30例,对照组(来曲唑组)于月经周期第3~5天起口服来曲唑,治疗组(右归丸联合来曲唑组)在对照组的基础上联用右归丸汤剂,2组均治疗3个月经周期。比较2组治疗后临床疗效、卵泡发育情况、子宫内膜容受性相关指标。结果(1)治疗组总有效率及妊娠率均大于对照组(均P<0.05)。(2)治疗组优势卵泡发育率高于对照组(P>0.05),治疗组周期排卵率优于对照组(P<0.05)。(3)比较2组子宫内膜容受性相关指标[HCG日子宫内膜厚度、A型子宫内膜率、子宫动脉血流阻力指数(RI)],治疗组均优于对照组,差异有统计学意义(均P<0.05)。结论加减右归丸能有效改善肾阳虚症状,促进卵泡发育及提高子宫内膜容受性,减少单纯用西药带来的不良反应,从而提高妊娠率。
Objective To observe the effect of Yougui Pill(右归丸)on follicular development and endometrial receptivity in patients with polycystic ovary syndrome(PCOS)infertility due to kidney yang deficiency.Methods 60 patients who met the enrollment requirements were randomly divided into 2 groups with 30 cases in each group.The control group(letrozole group)was given letrozole from the 3 rd to 5 th day of the menstrual cycle,and the treatment group(Yougui Pill combined with letrozole group)combined with Yougui Pill decoction on the basis of the control group.Both groups were treated for 3 menstrual cycles.The clinical efficacy,follicular development,and endometrial receptivity related indexes after treatment were compared between the two groups.Results(1)The total efficiency and pregnancy rate of the treatment group were higher than those of the control group(both P<0.05).(2)The development rate of dominant follicles in the treatment group was higher than that of the control group,the difference was not statistically significant(P>0.05),and the cycle ovulation rate in the treatment group was better than the control group(P<0.05).(3)Comparing the related indexes of endometrial receptivity between the two groups of patients,the treatment group was better than the control group[(endometrial thickness on HCG day,type A endometrial rate,uterine artery blood flow resistance index(RI)],the difference with statistical significance(P<0.05).Conclusion Modified Yougui Pill can effectively improve the symptoms of kidney-Yang deficiency,promote follicular development and improve endometrial receptivity,reduce the adverse reactions caused by the use of western medicine alone,thereby increasing the pregnancy rate.
