摘要
目的分析个案管理联合阶段健康教育对非霍奇金淋巴瘤(NHL)的干预效果。方法选取我院收治的NHL患者108例,使用随机数字表法分为常规组(常规治疗+护理)和联合组(常规治疗、护理联合个案管理和阶段健康教育),每组各54例,比较两组患者干预前后自我管理能力、化疗依从性、心理状态、生活质量等情况,并记录患者化疗相关不良反应。结果干预后联合组患者自我管理能力测定量表(ESCA)总分及各分项评分、治疗依从性、心理弹性量表(CD-RISC)、欧洲癌症治疗与研究组织的生活质量问卷(QLQ-C30)各维度评分及总分均高于常规组(P<0.05);焦虑自评量表(SAS)及抑郁自评量表(SDS)评分均低于常规组(P<0.05);化疗期间各不良反应发生率均低于常规组(P<0.05)。结论个案管理联合阶段健康教育可明显提高NHL患者自我管理能力、化疗依从性、心理弹性、生活质量,减少焦虑抑郁等负性情绪及化疗相关不良反应,值得在临床推广实践。
Objective To analyze the intervention effect of case management combined with stage health education on non-Hodgkin′s lymphoma(NHL).Methods A total of 108 NHL patients admitted to our hospital were selected and randomly divided into a conventional group(conventional treatment and nursing)and a combined group(conventional treatment and case management combined with stage health education),54 in each group.Self-management ability,chemotherapy compliance,psychological states,and quality of life were compared before and after intervention between the two groups.Chemotherapy-associated adverse reactions were also recorded.Results After intervention,the total score of Exercise of Self-Care Agency(ESCA)and scores of different items,chemotherapy compliance scores,Connor-Davidson Resilience scale(CD-RISC)scores,scores of different items in the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire-C30(QLQ-C30)and the total score of the combined group were higher than those of the conventional group(P<0.05).After intervention,the Self-rating Anxiety Scale(SAS)scores and Self-rating Depression Scale(SDS)scores in the combined group were lower than those in the conventional group(P<0.05).The incidence rates of adverse reactions during chemotherapy of the combined group were lower than those of the control group(P<0.05).Conclusion The self-management ability,chemotherapy compliance,psychological resilience,and quality of life of patients with NHL can be improved by case management combined with stage health education.It also reduces the negative emotions and chemotherapy-associated adverse reactions.It is worth promoting for clinical practice.
