整合医学视角下儿科专业消化教学的“整”与“合”

Integration and Combination of Digestive Teaching in Pediatrics From the Perspective of Integrated Medicine

摘要 2019年起上海交通大学医学院(交医)正式以儿科学专业“5+3”全新模式招生,采用与国际接轨“以器官为中心的整合医学”模式,结合基础医学、成人消化和儿童消化等多方面领域教(医)师共同授课及实践带教。我国医学体制与欧美国家差异很大,不能简单照搬,因此制定具有“我国特点、儿科特色”的整合医学课程势在必行。儿科专业既是老学科又是新晋专业,近年来儿童消化疾病谱有所改变,儿童肝病(肝豆状核变性、脂肪肝等)及炎症性肠病、消化道异物发病有所增多,其诊断及诊治依赖于内镜等辅助检查;鉴于部分小儿疾病会在成人阶段延续,实行“儿童—青少年—成人”的序贯诊治十分重要。文章将探讨在现代医学的背景下,根据儿科专业特点,如何通过“整”与“合”做到消化教学“基础与临床”“儿童与成人”和“理论与实践”的交互交融并高效地建立整合教学体系,以完善消化整合教学的内涵,推动整合课程教改。 Shanghai Jiao Tong University school of medicine has officially enrolled in the "5+3" new mode of Pediatrics since 2019,adopting the mode of "organ centered integrated medicine" in line with the international standards,and combining with the teaching(clinical) teachers in many fields such as basic medicine,adult digestion and children’s digestion to teach together and practice teaching.The medical system of China is very different from that of European and American countries,which can not be simply copied.Therefore,it is imperative to develop an integrated medical curriculum with "Chinese characteristics and pediatric characteristics".Pediatrics is both an old discipline and a new specialty.In recent years,the spectrum of digestive diseases in children has changed,and the incidence of liver diseases(hepatolenticular degeneration,fatty liver,etc.) and inflammatory bowel disease,foreign bodies in digestive tract has increased.Its diagnosis and treatment rely on endoscopy and other auxiliary examinations.In view of the fact that some children’s diseases will continue in the adult stage,it is very important to implement the sequential diagnosis and treatment of "Children Adolescent adult".In this paper,we will discuss how to achieve the interaction and integration of digestion teaching through "integration" and "combination" under the background of modern medicine and according to the characteristics of Pediatrics,so as to improve the connotation of digestive integration teaching and promote the integrated curriculum reform.
