摘要
目的探究小青龙汤联合玉屏风治疗肺脾气虚型鼻鼽的临床效果。方法随机数字表分组法将88例肺脾气虚型鼻鼽患者均分为2组。对照组服用氯雷他定片,观察组在对照组基础上用玉屏风颗粒和小青龙汤剂。对比2组主要主要症状体征评分、辅助T细胞17/调节T细胞(Th17/Treg)、血清总免疫球蛋白E(TIgE)、肿瘤坏死因子(TNF)-γ、白细胞介素(IL)-4、辅助T细胞1/辅助T细胞2(Th1/Th2)、临床总体效果和生活质量评分。结果与对照组比,观察组治疗后喷嚏、流涕、鼻塞、鼻痒、鼻黏膜水肿评分明显下降(P<0.01),外周血中Th17/Treg细胞比例、TIgE、IL-4水平均显著下降(P<0.01),TNF-γ水平和Th1/Th2比值显著升高(P<0.01),临床总体效果更优,过敏性鼻炎生活质量测定量表(RQLQ)评分显著下降(P<0.01)。结论小青龙汤联合玉屏风颗粒治疗肺脾气虚型鼻鼽疗效显著,能有效纠正Th17/Treg细胞比例和Th1/Th2细胞因子失衡,缓解鼻腔黏膜部位炎症性反应,从而改善患者临床症状和生活质量。
Objective To explore the clinical effect of Xiaoqinglong Decoction combined with Yupingfeng in the treatment of allergic rhinitis with deficiency of lung and spleen qi.Methods 88 patients with allergic rhinitis due to deficiency of lung and spleen qi were selected and divided into two groups by random number table method.The control group was treated by loratadine,and the observation group was treated by Yupingfeng granules,Xiaoqinglong decoction and loratadine.The main traditional Chinese medicine(TCM)syndrome scores,Th17/Treg,Total serum immunoglobulin E(TigE),tumor necrosis factor(TNF)-γ,interleukin(IL)-4,Th1/Th2,overall clinical effects and quality of life scores were compared between the two groups.Results Compared with the control group,the scores of sneezing,runny nose,nasal congestion,nasal itching,and nasal mucosal edema in the observation group decreased after treatment(P<0.01),and the ratio of Th17/Treg cells in peripheral blood,TIgE and IL-4 levels decreased(P<0.01),the TNF-γlevel and Th1/Th2 ratio increased(P<0.01),the overall clinical effect was better,and the rhinoconjunctivitisquality oflife questionnaire(RQLQ)score decreased(P<0.01).Conclusion Xiaoqinglong Decoction combined with Yupingfeng Granules is effective in treating allergic rhinitis with deficiency of lung and spleen qi,and can effectively correct the Th17/Treg cell ratio and Th1/Th2 cytokine imbalance,relieve the inflammatory reaction of the nasal mucosa,and improve the clinical symptoms and quality of life of the patients.
