中小学女生及其母亲人乳头瘤病毒疫苗接种现况和家长对疫苗接种的健康信念调查 被引量:8

摘要 目的 评价中小学女生及其母亲人乳头瘤病毒(Human papillomavirus, HPV)疫苗接种率和家长对接种HPV疫苗的健康信念。方法 采用多阶段随机抽样方法,于2019年7-10月在中国4个省8所中小学抽取9-18岁女生的家长开展问卷调查,分析女生及其母亲HPV疫苗接种率、家长对接种HPV疫苗的健康信念得分及其影响因素。结果 共调查5 203名中小学女生家长,中小学生及其母亲HPV疫苗接种率分别为1.33%和2.48%;在不愿为女生接种HPV疫苗的2 791名家长中,不了解HPV疫苗、担心疫苗安全、认为HPV疫苗价格昂贵分别占73.41%、44.82%、33.25%;家长对接种HPV疫苗的健康信念总平均得分为48.52±6.03分,易感性认知、严重性认知、利益性认知、障碍性认知和行动线索5个维度平均得分分别为3.27±0.82分、3.95±0.67分、3.90±0.64分、3.87±0.74分、3.74±0.51分;多元线性回归分析显示,年龄、职业、文化程度、家庭人均月收入和家族相关癌症史显著影响家长的健康信念总平均得分。结论 研究地区中小学女生及其母亲HPV疫苗接种率较低,家长对接种HPV疫苗的健康信念总水平较高而易感性认知不足;需加强HPV疫苗相关知识宣传,提高HPV疫苗接种率。 Objective To evaluate coverage levels of human papillomavirus(HPV) vaccine among primary and secondary school girls and their mothers and to determine parental health beliefs about HPV vaccination.Methods We used a multi-stage random sampling method to select parents of 9-18-year-old girls from 8 primary and secondary schools in 4 provinces of China during July-October 2019 for a questionnaire-based survey. We analyzed HPV coverage among girls and their mothers, and we assessed parents’ health beliefs about HPV vaccination and factors influencing health beliefs.Results A total of 5 203 parents of primary and secondary school girls were included. Coverage levels of HPV vaccine were 1.33% and 2.48% among girls and their mothers, respectively. Among the 2 791 parents who were not willing to vaccinate girls with HPV vaccine, 73.41% did not know about HPV vaccine, 44.82% worried about vaccine safety, and 33.25% believed HPV vaccine was expensive. The overall mean score of parents’ health belief about HPV vaccination was 48.52±6.03, with mean scores for its five dimensions-susceptibility, seriousness, benefits, barriers, and cues to action being 3.27±0.82, 3.95±0.67, 3.90±0.64, 3.87±0.74, and 3.74±0.51, respectively. Multiple linear regression analysis showed that age, occupation, education level, per capita monthly family income, and family cancer history significantly influenced the overall mean score of parents’ health beliefs.Conclusions Coverage levels of HPV vaccine were low among primary and secondary school girls and their mothers. The overall level of parents’ health beliefs about HPV vaccination was high while their awareness of susceptibility was low. We should further publicize HPV vaccine knowledge to increase vaccine coverage.
