TI-RADS分级+CEUS检查对甲状腺癌的诊断价值 被引量:1

摘要 目的:探讨二维超声甲状腺影像报告和数据系统(TI-RADS)分级+超声造影(CEUS)检查对甲状腺癌的诊断价值。方法:回顾性分析我院129例甲状腺结节患者2018年6月至2020年6月的临床资料,均经手术病理检查明确诊断,均行TI-RADS、CEUS检查,比较良恶性甲状腺结节TI-RADS分级、CEUS评分及影像特征,并比较TI-RADS分级、CEUS及TI-RADS分级+CEUS检查对甲状腺癌的诊断价值。结果:129例患者中共检出145个甲状腺结节,其中103个恶性、42个良性;恶性甲状腺结节TI-RADS分级评分高于良性甲状腺结节,成分为囊实性或实性、形态为高>宽、回声为低回声、边缘为分叶或不规则、局灶强回声为点状强回声等超声特征表现比例多于良性甲状腺结节(P<0.05);恶性甲状腺结节CEUS评分高于良性甲状腺结节,低增强、向心性增强、不均匀增强、增强后边界不清晰、周边未环状增强等CEUS特征表现比例多于良性甲状腺结节(P<0.05);受试者工作特征曲线显示TI-RADS分级+CEUS检查的曲线下面积大于TI-RADS分级、CEUS检查。结论:CEUS、TI-RADS分级检查应用于甲状腺结节患者良恶性判断中均可根据影像特征判断其性质,且二者联合能提高甲状腺癌的诊断效能。 Objective:To explore the diagnostic value of two-dimensional ultrasonic thyroid image report and data system(TI-RADS)grading combined with contrast-enhanced ultrasound(CEUS)in thyroid cancer.Methods:The clinical data of 129 patients with thyroid nodules in our hospital from Jun 2018 to Jun 2020 were retrospectively analyzed.The diagnosis of patients was confirmed by surgical and pathological examination.TI-RADS and CEUS were carried out.The TI-RADS grading,CEUS score and imaging characteristics of benign and malignant thyroid nodules were compared.The diagnostic value of TI-RADS grading,CEUS and TI-RADS grading+CEUS in thyroid cancer were compared.Results:A total of 145 thyroid nodules were detected in 129 patients,among which 103 were malignant and 42 were benign.The TI-RADS score of malignant thyroid nodules was higher than that of benign thyroid nodules,and the proportion of ultrasonographic features such as cystic solid component,solid component,high>width morphology,low echo,lobulated or irregular edge,focal strong echo with point-like strong echo was higher than that of benign thyroid nodules(P<0.05).The CEUS score of malignant thyroid nodules was higher than that of benign thyroid nodules,and the proportion of CEUS features such as low enhancement,concentric enhancement,uneven enhancement,unclear boundary after enhancement,and peripheral non-annular enhancement was higher than that of benign thyroid nodules(P<0.05).The ROC curve showed that the AUC value of TI-RADS grading+CEUS was higher than that of TI-RADS grading and CEUS examination.Conclusion:CEUS examination and TI-RADS grading can be used in the diagnosis of benign and malignant thyroid nodule according to the image characteristics,and the combination of CEUS and TI-RADS grading can improve the diagnostic efficiency of thyroid cancer.
