抗血管内皮生长因子治疗糖尿病黄斑水肿患者短期疗效影响因素分析 被引量:3

Analysis of influencing factors on short-term efficacy of anti-vascular endothelial growth factor therapy in patients with diabetic macular edema

摘要 目的观察抗血管内皮生长因子(VEGF)玻璃体内注射治疗糖尿病黄斑水肿(DME)患者短期疗效的影响因素。方法回顾性分析。纳入接受玻璃体内注射抗VEGF药物的DME患者67例67眼,采集患者一般信息、全身病史及相关血液学检测指标(血糖、血常规、血脂、生化、肾功能等)、眼部病史及眼科相关检查结果视力、眼压、眼底彩色照相、OCT相关影像学指标、荧光素眼底血管造影(FFA)等。采用单因素及二元logistic回归分析进行数据统计。结果将单因素分析结果中有统计学差异的指标纳入二元logistic回归分析,结果显示,基线1 mm黄斑中心视网膜体积小(OR=0.001,95%CI为0.001~0.036)、1 mm黄斑中心视网膜厚度薄(OR=0.994,95%CI为0.988~1.000)、白细胞计数低(OR=0.561,95%CI为0.332~0.948)、血红蛋白浓度高(OR=1.076,95%CI为1.021~1.135)、红细胞计数高(OR=11.798,95%CI为1.854~75.078)、视力下降时间长(OR=2.008,95%CI为1.053~3.830)、基线最佳矫正视力低(OR=0.274,95%CI为0.093~0.804)是DME患者抗VEGF治疗后短期疗效不佳的独立影响因素。结论DME患者基线时视功能与眼部解剖结构以及全身情况可能影响抗VEGF治疗的短期疗效。 Objective To explore the short-term influencing factors of intravitreal injection of anti-vascular endothelial growth factor(VEGF)drugs in the treatment of diabetic macular edema(DME).Methods In this retrospective study,a total of 67 patients(67 eyes)with DME who received intravitreal injection of anti-VEGF drugs were enrolled.General information,systemic medical history,related hematological indexes(blood glucose,blood routine,blood lipids,biochemistry,and renal function),eye disease history,and eye examination results(visual acuity,intraocular pressure,fundus color photography,optical coherence tomography related imaging indicators,and fundus fluorescein angiography)of patients were collected.Univariate and binary logistic regression analysis were used for data statistics.Results The indexes with statistical difference in the univariate analysis results were included in the binary logistic regression analysis.The results showed smaller 1 mm TMV(total macular volume)(OR=0.001;95%CI 0.001-0.036),thinner 1 mm CMT(central macular thickness)(OR=0.994;95%CI 0.988-1.000),lower white blood cell count(OR=0.561;95%CI 0.332-0.948),higher hemoglobin concentration(OR=1.076;95%CI 1.021-1.135),higher red blood cell count(OR=11.798;95%CI 1.854-75.078),longer periods of vision loss(OR=2.008;95%CI 1.053-3.830),and lower best corrected visual acuity(OR=0.274;95%CI 0.093-0.804)at baseline were independent factors resulting in poor short-term effect after anti-VEGF treatment in DME patients.Conclusion The ocular visual function,anatomical structure,and systemic conditions of DME patients at baseline may affect the short-term effect of anti-VEGF treatment.
