摘要
目的研究2型糖尿病患者PBG、FINS、HbA1c水平与早期糖尿病肾病发生的关系。方法选取2018年5月—2020年5月收治的96例2型糖尿病作为研究组,根据尿白蛋白排泄率(UAER)水平分为2型糖尿病组(UAER<30μg/min,n=53)和早期糖尿病肾病组(30μg/min≤UAER<300μg/min,n=43);另选取同期体检健康者87例作为对照组。比较研究组和对照组、糖尿病组和早期糖尿病肾病组PBG、FINS、HbA1c水平,分析影响糖尿病肾病发生的危险因素;受试者工作特征曲线分析PBG、FINS、HbA1c对早期糖尿病肾病的预测价值。结果研究组PBG、FINS、HbA1c水平高于对照组(P<0.01)。早期糖尿病肾病组PBG、FINS、HbA1c水平高于2型糖尿病组(P<0.01)。PBG、FINS、HbA1c升高为影响糖尿病肾病发生的独立危险因素(P<0.01)。PBG、FINS、HbA1c联合检测对早期糖尿病肾病预测价值的曲线下面积更高,为0.877。结论PBG、FINS、HbA1c是影响2型糖尿病患者发生糖尿病肾病的独立危险因素,且联合检测PBG、FINS、HbA1c可作为预测早期糖尿病肾病的有效指标。
Objective To study the relationships between the levels of PBG,FINS,HbA1c and the occurrence of early diabetic nephropathy(EDN)in patients with type 2 diabetes mellitus(T2DM).Methods A total of 96 patients with T2DM who were treated from May 2018 to May 2020 were selected as the research group.According to the level of urinary albumin excretion rate(UAER),they were divided into T2DM group(UAER<30μg/min,n=53)and EDN group(30μg/min≤UAER<300μg/min,n=43);87 healthy subjects who underwent physical examination during the same period were selected as control group.The levels of PBG,FINS and HbA1c were compared between the research group and the control group,and between the T2DM group and the EDN group,and the risk factors for diabetic nephropathy were analyzed.Receiver operating characteristic(ROC)curve was used to analyze the predictive value of PBG,FINS and HbA1c for EDN in patients with T2DM.Results The levels of PBG,FINS and HbA1c in the research group were higher than those in the control group(P<0.01).The levels of PBG,FINS and HbA1c in the EDN group were higher than those in the T2DM group(P<0.01).Increased PBG,FINS and HbA1c were independent risk factors for occurrence of diabetic nephropathy in patients with T2DM(P<0.01).The area under the ROC curve of the combined detection of PBG,FINS and HbA1c for predicting EDN was higher,which was 0.877.Conclusion PBG,FINS and HbA1c are independent risk factors for diabetic nephropathy in patients with T2DM,and combined detection of PBG,FINS and HbA1c can be used as indicators to predict the occurrence of EDN.
