摘要
目的观察常规超声(二维超声、彩色多普勒超声)联合二维剪切波弹性成像(2D-SWE)评价2型糖尿病(T2DM)患者涎腺异常的价值。方法将65例T2DM患者分为A组(病程<10年,n=30)和B组(病程≥10年,n=35);另以同期30名健康志愿者为对照组。对比观察3组涎腺(腮腺、下颌下腺)超声评分、腺体面积、血流信号及杨氏模量值(E),包括单侧腮腺及下颌下腺E最大值(E_(max))、平均值(E_(mean))、最小值(E_(min))及两侧平均值E_(总max)、E_(总mean)及E_(总min);分析常规超声联合2D-SWE评价T2DM涎腺异常的价值。结果3组涎腺超声评分、面积及血流信号数目差异均有统计学意义(P均<0.05);A、B组腮腺及下颌下腺超声评分、面积及涎腺总超声评分均大于对照组而血流信号数目均小于对照组(P均<0.05),且A组腮腺及下颌下腺面积均小于B组(P均<0.05)。3组间涎腺E_(总max)、E_(总mean)及E_(总min)差异均有统计学意义(P均<0.05);其中A、B组涎腺E_(总max)、E_(总mean)及E_(总min)均高于对照组(P均<0.05),且A组涎腺E_(总max)、E_(总mean)及E_(总min)均低于B组(P均<0.05)。结论常规超声联合2D-SWE可评价T2DM患者涎腺异常改变,包括腺体面积、超声评分及硬度均增加及血流信号减少。
Objective To observe the value of conventional ultrasound(two-dimensional and color Doppler ultrasonography)combined with two-dimensional shear wave elastography(2D-SWE)for evaluating abnormalities of salivary glands in patients with type 2 diabetes mellitus(T2DM).Methods Totally 65 T2DM patients were enrolled and divided into group A(course of disease<10 years,n=30)and B(course of disease≥10 years,n=35),while 30 healthy volunteers in the same period were taken as controls(control group).The ultrasound score,area and blood flow single of salivary glands(parotid and submandibular glands)were observed and compared among 3 groups,as well as Young's modulus(E),including the maximum E(E_(max)),mean E(E_(mean))and minimum E(E_(min))of unilateral parotid and submandibular glands and the bilateral mean values of above indexes,i.e.E_(total max),E_(total mean)and E_(total min).The value of conventional ultrasound combined with 2D-SWE for evaluating abnormalities of salivary glands in patients with T2DM was analyzed.Results Significant differences of ultrasound scores,areas and blood flow signals of salivary glands were found among group A,B and control group(all P<0.05).The ultrasound scores and areas of parotid and submandibular glands,as well as the total ultrasound scores of salivary glands of group A and B were all greater than those of control group,while blood flow signals was less than that of control group(all P<0.05),and the area of parotid and submandibular glands of group A was less than that of group B(all P<0.05).There were significant differences of E_(total max),E_(total mean)and E_(total min)of salivary glands among 3 groups(all P<0.05).E_(total max),E_(total mean)and E_(total min)of salivary glands of group A and B were higher than those of control group,and E_(total max),E_(total mean)and E_(total min)of salivary glands of group A were lower than those of group B(all P<0.05).Conclusion Conventional ultrasound combined with 2D-SWE could evaluate abnormalies of salivary glands in T2DM patients,mainly manifested as increased area,ultrasound score and hardness and decreased blood flow signals of glands.
