微柱凝胶法交叉配血技术中供血者意外抗体筛查的临床应用效果 被引量:3

摘要 目的分析微柱凝胶法交叉配血技术中供血者意外抗体筛查对于输血安全的重要作用。方法选取医院在2017年1月至2020年2月行治疗性输血和手术备血的患者596例,应用微柱凝胶法对受血者予以血清中不规则抗体筛查,筛查阳性标本进行抗体特异性测定。结果596例受血人员共测定出抗体阳性240例(40.27%)。选取血型系统Rh-hr、MNSs、Kidd、Duffy、Kell、Lewis。240例阳性标本不规则抗体中,164例(68.33%)女性、76例(31.67%)男性,其中190例(79.17%)输血史/妊娠史、50例(20.83%)无输血史/妊娠史。240例抗体筛选阳性标本不规则抗体记录结果显示:测定出特异性抗体共106例(44.17%),未测定出抗体共134例(55.83%)。Rh-hr血型系统中,8例(3.33%)抗-D抗体、34例(14.17%)抗-E抗体、2例(0.83%)抗-C抗体、4例(1.67%)抗-C抗体、2例(0.83%)抗-Ec抗体。MNSs血型系统中,34例(14.17%)抗-M抗体、4例(1.67%)抗-s抗体。Kidd血型系统中,2例(0.83%)抗-JK抗体。Duffy血型系统中,2例(0.83%)抗-Fy抗体。Kell血型系统中,2例(0.83%)抗-K抗体。Lewis血型系统中,6例(2.50%)抗-K抗体、2例(0.83%)抗-Le^(b)抗体、4例(1.67%)自身抗体。结论在输血前测定不规则抗体是确保输血安全的主要方式,微柱凝胶法应用简便,且灵敏度较高,结果更具有稳定性,适用于输血前不规则抗体血清学测定。 Objective To analyze the important role of accidental antibody screening in blood donors for the safety of blood transfusion in microcolumn gel cross-matching technique.Methods From January 2017 to February 2020,596 patients who underwent therapeutic blood transfusion and surgical blood preparation were selected from a selected hospital.Microcolumn gel method was used to screen for irregular antibodies in the serum of the blood recipients,and the positive samples were screened for antibody specificity.Results A total of 240 cases(40.27%)of 596 blood samples were detected with positive antibody.The blood group systems RH-HR,MNSs,Kidd,Duffy,Kell,Lewis were selected.Among the 240 positive specimens with irregular antibodies,164 cases(68.33%)were female and 76(31.67%)cases were male,among which 190cases(79.17%)had a history of blood transfusion/pregnancy and 50 cases(20.83%)had no history of blood transfusion/pregnancy.The irregular antibody records of 240 positive samples showed that 106 cases(44.17%)had specific antibodies and 134 cases(55.83%)had no antibodies.In RHHR blood group system,there were 8 cases(3.33%)of anti-D antibody,34 cases(14.17%)of anti-E antibody,2 cases(0.83%)of anti-C antibody,4 cases(1.67%)of anti-C antibody and 2 cases(0.83%)of anti-EC antibody.There were 34 cases(14.17%)of anti-M antibody and 4 cases(1.67%)of anti-S antibody in MNSs blood group system.In Kidd blood group system,2 cases(0.83%)had anti-JK antibody.In Duffy blood group system,2 cases(0.83%)had anti-FY antibody.In Kell blood group system,anti-K antibody was found in 2 cases(0.83%).In Lewis blood group system,there were 6 cases(2.50%)of anti-K antibody,2 cases(0.83%)of anti-LEB antibody and 4 cases(1.67%)of autoantibody.Conclusion The determination of irregular antibodies before blood transfusion is the main way to ensure the safety of blood transfusion.Microcolumn gel method is simple and sensitive,and the result is more stable.It is suitable for serological determination of irregular antibodies before blood transfusion.
