主成分拟合模型用于护士睡眠影响因素的研究

摘要 目的:了解某院护士睡眠质量现状及影响因素的综合作用。方法:调查某院184名护士,采集护士人口学资料,采用护理工作环境量表(Practice Environment Scale,PES)、心理资本量表(PsyCap Questionnaire,PCQ)、护士工作投入量表(Job involvement scale,UWES)、匹兹堡睡眠质量指数(Pittsburgh sleep quality index,PSQI)进行评分。单因素分析各因素对PSQI指数的具体影响,用主成分分析的方法对影响因素进行降维,提取公因子,分析公因子与影响因素的内部联系。以PSQI指数为因变量,分别以主成分或影响因素为自变量,拟合回归模型,比较两种模型的差异。结果:184名护士PSQI指数(13.06±6.30),年龄35岁及以上、家庭月收入2万以下、受教育年限15年及以下、护师及以下、已婚、职业价值较低、同事支持较少、家庭影响较多、薪资待遇较差、职业倦怠较多护士得分更高(P<0.05)。职业发展、领导与管理、医护关系、认可氛围、专业自主、充足的人力、自我效能、乐观、希望、坚韧、专注评分越低,PSQI指数越高(P<0.05)。主成分分析获得6个主成分,贡献率从大到小分别是:客观因素干扰(22.410%)、人力资源配置(20.148%)、岗位认可程度(18.702%)、应激处事能力(14.553%)、职业发展现状(11.677%)、科室人文管理(10.513%)。分别以主成分或影响因素为自变量,拟合两个模型,主成分线性回归模型(VIF=3.135,)优于多因素线性回归模型(VIF=87.246)。结论:护士睡眠质量相对较差,影响其睡眠质量的因素众多,客观因素干扰和人力资源配置不足可能为主要原因。提取主成分构建模型的方法,有助于对护士睡眠质量影响因素进行真实的归纳。 Objective:To understand the status of sleep quality of nurses in acertain hospital and the comprehensive influence of influencing factors.Methods:A total of 184 nurses in our hospital were investigated.including demographic data,PES,self-capacity(PCQ and UWES),external influences,and sleep quality(PSQI).Single factor analysis was carried out to analyze the specific influence of each factor on PSQI index.Principal component analysis was used to reduce the dimension of influencing factors,extract common factors,and analyze the internal relation between common factors and influencing factors.With PSQI as the dependent variable and principal components or influencing factors as independent variables,the regression model was fitted and the differences between the two models were compared.Results:PSQI index of 184 nurses(13.06±6.30),age 35 and above,family monthly income less than20,000 yuan,education years less than 15 years,nurse or below,married,low professional value,less support from colleagues,more family influence,poor salary,more job burnout score higher(P<0.05).The lower the score of career development,leadership and management,healthcare relationship,recognition atmosphere,professional autonomy,sufficient manpower,self-efficacy,optimism,hope,tenacity,and focus,the higher the PSQI index(P<0.05).The principal component analysis obtained five principal components,and the contribution rates from large to small were:interference of objective factors(22.410%),human resource allocation(20.148%),job recognition(18.702%),stress handling ability(14.553%),career development status(11.677%),department humanistic management(10.513%).The two models were fitted with principal components or influencing factors as independent variables.The principal component linear regression model(VIF=3.135)was superior to the multi-factor linear regression model(VIF=87.246).Conclusion:The sleep quality of nurses is relatively poor,and there are many factors affecting their sleep quality.Objective factors and insufficient allocation of human resources may be the main reasons.The method of constructing the model by extracting the principal components is helpful to conclude the factors affecting the sleep quality of nurses.
