摘要
目的建立由手术室护士主导的剖宫产术中新生儿断脐护理模式并应用于临床,以期在同类型医院手术室推广。方法创建手术室护士主导的剖宫产术中新生儿断脐相关制度及流程规范,组织相关培训及工作坊,规范新生儿断脐技术、体格检查、文书书写及复苏抢救配合相关事项,组建手术室特色新生儿断脐护理团队。经过2年的实践,统计该模式实施后手术室护士相关理论及操作考核成绩、不良事件发生率、医生满意度、手术室护士自身反馈满意度。结果更改模式后,手术室护士新生儿断脐、复苏抢救相关理论及操作考核成绩,合格率均为100%,2019、2020、2021年不良事件发生率分别为0.38%、0.36%、0.27%,医生、手术室护士自身对该模式的满意率分别为91%、87%。结论该模式实现手术室内部闭环管理,利于护理管理者更科学合理的分配人员岗位,控制院感发生、缩短手术非必要等待时长,利于急诊、绿色通道等手术的进行。此模式更加契合临床需求,开拓护士新岗位新技能,提升护士职业成就感。
Objective To establish a nursing mode of neonatal umbilical cord cutting during cesarean section led by operating room nurses,and to promote the mode in the operating room of the same type of hospital.Methods The system and process specification for neonatal umbilical cord cutting during cesarean section led by operating room nurses were developed and identified;relevant training and workshops were conducted and organized for standardizing neonatal umbilical cord cutting technique,physical examination,writing of documents,and coordinating with resuscitation and rescue.A neonatal umbilical cord cutting team with the characteristics of operating room was established.After more than two years of practice,the performance of operating room nurses in related theoretical and operational assessment,the incidence of adverse events,doctor satisfaction,and the satisfaction of operating room nurses were observed.Results After the implementation of the new mode,the pass rates in the theoretical and operational assessments regarding umbilical cord cutting,resuscitation and rescue were all 100%,and the incidence of adverse events from 2019 to 2021 were 0.38%,0.36%,and 0.27%,respectively.The satisfaction rate of the operating room nurses was 91%and 87%,respectively.Conclusion The internal closed loop management of the operating room is achieved through the new mode,which is conducive to the scientific allocation of personnel resources,reduced occurrence of hospital infection,and shortened waiting time for surgery,and it is conducive to emergency and green channel operations.This mode is in line with clinical needs,helps to develop new positions and new skills for nurses,and enhance nurses’sense of professional achievement.
