经鼻持续气道正压通气对儿童重症肺炎合并呼吸衰竭的治疗价值 被引量:5

摘要 目的分析对儿童重症肺炎合并呼吸衰竭实施经鼻持续气道正压通气治疗的价值。方法80例重症肺炎伴呼吸衰竭患儿,根据随机抽签法分为实践组与参考组,各40例。参考组予以经鼻导管吸氧治疗,实践组予以经鼻持续气道正压通气治疗。对比两组治疗前后的血气指标、治疗效果、不良事件发生率。结果治疗后,实践组pH(7.40±0.15)、氧分压(PaO_(2))(82.37±17.45)mm Hg(1 mm Hg=0.133 kPa)高于参考组的(7.31±0.14)、(65.41±14.62)mm Hg,二氧化碳分压(PaCO_(2))(46.38±7.27)mm Hg低于参考组的(50.81±10.25)mm Hg,差异有统计学意义(P<0.05)。实践组治疗总有效率为95.00%,高于参考组的77.50%,差异有统计学意义(P<0.05)。实践组不良事件发生率为5.00%,低于参考组的20.00%,差异有统计学意义(P<0.05)。结论经鼻持续气道正压通气治疗重症肺炎伴呼吸衰竭疗效显著,可有效改善患儿血气指标和临床症状,值得应用与推广。 Objective To analyze the value of nasal continuous positive airway pressure in children with severe pneumonia complicated with respiratory failure.Methods A total of 80 children with severe pneumonia and respiratory failure were randomly divided into practice group and reference group,with 40 cases in each group.The reference group was treated with oxygen inhalation via nasal catheter,and the practice group was treated with nasal continuous positive airway pressure.The blood gas indicators before and after treatment,therapeutic effect and incidence of adverse events were compared between the two groups.Results After treatment,the pH(7.40±0.15),arterial partial pressure of oxygen(PaO_(2))(82.37±17.45)mm Hg(1 mm Hg=0.133 kPa)in the practice group were higher than(7.31±0.14),(65.41±14.62)mm Hg in the reference group,and the arterial partial pressure of carbon dioxide(PaCO_(2))(46.38±7.27)mm Hg was lower than(50.81±10.25)mm Hg in the reference group.All the differences were statistically significant(P<0.05).The total effective rate of the practice group was 95.00%,which was higher than 77.50%of the reference group,and the difference was statistically significant(P<0.05).The incidence of adverse events in the practice group was 5.00%,which was lower than 20.00%in the reference group,and the difference was statistically significant(P<0.05).Conclusion Nasal continuous positive airway pressure has a significant efficacy on the treatment of severe pneumonia with respiratory failure,which can effectively improve the blood gas indicators and clinical symptoms of children.It is worthy of application and promotion.
