摘要
目的:探讨2型糖尿病(T2DM)患者发生甲状腺功能减退(甲减)的影响因素。方法:选取80例T2DM患者为研究对象,依据是否发生甲减分为发生组与未发生组,统计两组基线资料和实验室指标水平,分析T2DM患者发生甲减的影响因素。结果:80例T2DM患者中,18例发生甲减,占22.50%(18/80),62例未发生甲减,占77.50%(62/80);发生组T2DM病程≥7年、HbA1c>9%、24 h尿微量白蛋白>30 mg/L占比均高于未发生组,差异有统计学意义(P<0.05);两组性别、年龄、用药史、合并高血压、合并冠心病、FBG≥8.0 mmol/L、2hPG≥11.1 mmol/L占比比较,差异均无统计学意义(P>0.05);Logistic回归分析结果显示,T2DM病程≥7年、HbA1c>9%和24 h尿微量白蛋白>30 mg/L均为影响T2DM患者发生甲减的危险因素(OR>1,P<0.05)。结论:T2DM病程≥7年、HbA1c>9%和24 h尿微量白蛋白>30 mg/L均为影响T2DM患者发生甲减的危险因素。
Objective:To investigate the influencing factors of hypothyroidism in patients with type 2 diabetes mellitus(T2DM).Methods:80 T2DM patients were selected as the research objects,and were divided into occurrence group and non-occurrence group according to whether hypothyroidism occurred or not.The baseline data and the related laboratory index levels of the two groups were statistically analyzed.Then,the influencing factors of hypothyroidism in the T2DM patients were analyzed.Results:Among the 80 T2DM patients,18 had hypothyroidism,accounting for 22.50%(18/80);62 did not have hypothyroidism,accounting for 77.50%(62/80).The proportion of the patients with T2DM duration≥7 years,HbA1c>9%,and 24-hour urinary microalbumin>30 mg/L in the occurrence group were higher than those in the non-occurrence group,and the difference was statistically significant(P<0.05).There were no significant differences in gender,age,comorbidity(hypertension,coronary heart disease),drug use(biguanide,insulin,sulfonylureas),FBG,and 2hPG between the two groups(P>0.05).Logistic regression analysis showed that the duration of T2DM≥7 years,HbA1c>9% and 24-hour urinary microalbumin>30 mg/L were risk factors for hypothyroidism in the T2DM patients(OR>1,P<0.05).Conclusions:T2DM≥7 years,HbA1c>9% and 24-hour urinary microalbumin>30 mg/L are the risk factors for hypothyroidism in the T2DM patients.
