摘要
目的 探讨血氧饱和度探头重置时间与医疗器械相关压力性损伤(MDRPI)阳性率之间的相关性,以为临床提供依据。方法 以便利抽样法,选取2020年1月至2021年3月浙江大学第一附属医院北仑分院收治的住院患者256例,以随机数字表法将256例患者均分为4组,每组64例,依次为A、B、C、D四组,4组患者的血氧饱和度监测仪重置时间分别为1、2、3、4 h,统计MDRPI发生情况,分析重置时间与MDRPI发生率之间的相关性。结果本次研究中MDRPI阳性率为10.55%,其中AB两组相比差异无统计学意义(P>0.05),BC、CD之间阳性率比较差异有统计学意义(P<0.05),本次研究中27例阳性患者,2期以上MDRPI发生率为22.22%,BC两组患者MDRPI 2期以上的发生率差异有统计学意义(P<0.05),Spearman相关性分析结果显示,患者血氧饱和度监测探头重置时间(h)与MDRPI阳性率之间存在正相关关系(r=2.338,P=0.013)。结论 血氧饱和度探头重置时间与MDRPI阳性率之间明显正相关,建议在临床工作中,将血氧探头每2 h改变一次部位,以规避血氧探头引起MDRPI的风险,减少医疗纠纷,提高医疗质量,提高患者满意度。
Objective To explore the correlation between the reset time of the blood oxygen saturation probe and the positive rate of medical device-related pressure injury(MDRPI) in order to provide evidence for clinical application.Methods By a convenience sampling method, 256 inpatients admitted to our hospital from January 2020 to March 2021 were selected. They were divided into 4 groups by random number method, with 64 cases in each group(Group A, B, C and D). The reset time of the pulse oximeter in the 4 groups of patients was 1 hour, 2 hours, 3 hours, and 4 hours,respectively. The occurrences of MDRPI were counted, and the correlation between the reset time and the incidence of MDRPI was analyzed. Results The positive rate of MDRPI in this study was 10.55%. There was no statistical difference in the positive rate between Group A and B(P>0.05), and there was significant difference in the positive rate between Group B and C or between Group C and D(P<0.05). For 27 patients with positive results in this study, the incidence of MDRPI above stage 2 was 22.22%. There was a significant difference in the incidence of MDRPI above stage 2 between Group B and C(P<0.05). The results of Spearman′s correlation analysis showed that the reset time of blood oxygen saturation probe(h) was positively correlated with the positive rate of MDRPI(r=2.338, P=0.013). Conclusion There is an obvious positive correlation between the reset time of the blood oxygen saturation probe and the positive rate of MDRPI. It is recommended to change the position of the blood oxygen saturation probe every 2 hours in clinical work in order to avoid the risk of MDRPI caused by the blood oxygen saturation probe, reduce medical disputes, and improve medical quality and patient satisfaction.
