胸部肿瘤放疗患者发生2级以上放射性肺炎的影响因素 被引量:2

摘要 目的分析胸部肿瘤患者在放射治疗中出现2级以上放射性肺炎(RP)的影响因素。方法收集从2016年1月至2019年1月在石家庄平安医院行胸部肿瘤放疗患者130例的临床资料、中药应用情况以及放疗计划系统(TPS)导出的放射治疗计划参数。分析其与患者发生2级以上RP的关系。以放射治疗开始3个月内出现2级以上RP为研究终点。采用logistic回归分析胸部肿瘤患者发生2级以上RP的影响因素。结果全组RP情况:2级以上发生率为25.38%(33/130),其中2级发生率为21.54%(28/130),3级发生率为3.85%(5/130),未出现4级RP。单因素分析显示,RP的发生在肺疾病史、总剂量、双肺V_(20)及是否应用中药方面差异有统计学意义(P<0.05)。多因素回归分析显示,肺病史、双肺V_(20)为发生2级以上RP的独立影响因素(均P<0.05)。结论在胸部肿瘤放射治疗中肺病史、双肺V_(20)是2级以上RP发生的独立危险因素。 Objective To analyze the influencing factors of grade 2 or above radiation pneumonia(RP)in patients with chest tumor during radiotherapy.Methods From January 2016 to January 2019,the clinical data,the application of traditional Chinese medicine and the parameters of radiotherapy plan derived from the treatment planning system(TPS)of 130 patients who received radiotherapy for thoracic tumor in Shijiazhuang Ping,an Hospital were collected,and analyzed the relationship between the patients and the grade 2 or above RP.The endpoint of the study was the occurrence of grade 2 or more RP within 3 months of the initiation of radiotherapy.Logistic regression was used to analyze the influencing factors of grade 2 or above RP in thoracic tumor patients.Results The incidence of grade 2 RP was 25.38%(33/130),of which grade 2 RP was 21.54%(28/130)and grade 3 RP was 3.85%(5/130).There was no grade 4 RP.Univariate analysis showed that there were significant differences in the occurrence of RP in lung disease history,total dose,bilateral lung V_(20) and whether traditional Chinese medicine was used(P<0.05).Multivariate regression analysis showed that lung disease history and bilateral lung V_(20) were the independent factors influencing the occurrence of grade 2 RP(P<0.05).Conclusion The history of lung disease and V_(20) in both lungs are independent risk factors for the occurrence of grade 2 or above RP in thoracic tumor radiotherapy.
