摘要
目的探究改进妇科恶性肿瘤腔镜手术体位摆放技术的效果。方法选取行腔镜手术治疗的妇科恶性肿瘤患者120例,随机分为对照组(传统截石位)及观察组(改进摆放体位),每组60例。对比2组麻醉前体位舒适度评分,术中一般情况(眼内压、平均动脉压),术后皮下气肿、肩痛及下肢疼痛麻木的发生率。结果患者手术体位摆放完成后,观察组舒适度高于对照组(P<0.05)。在气腹+体位摆放后30 min至240 min及恢复平卧位5 min时,观察组眼压较对照组降低,变化趋势相对稳定(P<0.05);观察组平均动脉压在麻醉+平卧位、气腹+体位摆放后5 min低于对照组(P<0.05);与对照组相比,观察组皮下气肿、肩痛及下肢疼痛发生率下降(P<0.05)。结论在妇科恶性肿瘤腔镜手术中运用改进的体位摆放技术可以增加患者舒适度,稳定患者术中一般情况,减少不良反应,具有推广价值。
Objective To explore the effect of improved posture placement technique in endoscopic surgery for gynecological malignant tumors.Methods 120 patients with gynecological malignant tumor were randomly divided into control group(traditional lithotomy position)and observation group(improved placement position),with 60 cases in each group.Preoperative comfort score,intraoperative general conditions(intraocular pressure,mean arterial pressure),postoperative incidence of subcutaneous emphysema,shoulder pain and lower limb pain and numbness were compared.Results After the operation position was placed,the comfort of the observation group was higher(z=3.899,P<0.05).When the gas belly+body is placed,the observation group was lowered by 5 min,and the observation group was lowered,and the change trend was relatively stable.There were statistical differences(P<0.05);Observation group average arterial pressure in anesthesia+Flat position,5 min after gas belly+position was significantly lower(P<0.05);Compared with the group,the epithelial imperimentalization,shoulder pain and lower extremitic pain incidence have been significantly reduced(P<0.05).Conclusion The application of improved posture placement technology in endoscopic surgery for gynecological malignant tumors can increase the comfort level of patients,stabilize the general conditions of patients during surgery,and reduce the incidence of postoperative adverse reactions,which is of popularization value.
